Summary
As a Support Worker at Eclipse Foundation for Individualized Supports you will play a crucial role in assisting individuals with disabilities to lead fulfilling lives within their communities. Your contributions will directly impact our mission of promoting inclusivity and providing essential support.
Responsibilities
- Provide compassionate assistance to individuals with disabilities in daily activities.
- Support clients in achieving their personal goals and enhancing their independence.
- Administer medications and manage health-related tasks as needed.
- Assist clients with personal care and daily living activities (ADLs).
- Collaborate with families and other professionals to implement individualized support plans.
- Maintain accurate documentation of client progress and needs.
- Facilitate social engagement and community involvement for clients.
- Ensure a safe and supportive environment for all clients.
Qualifications
- High School Diploma or equivalent
- Education and/or a minimum 2 years' experience in the field of Community Living, Social Services, Mental Health, Health Care, Employment Services, or related experience providing direct care to individuals living with barriers to inclusion in their community.
- Strong communication skills and the ability to build rapport with clients.
- Knowledge of medication administration protocols.
- Familiarity with ADLs and personal care practices.
- Compassionate, patient, and dedicated to enhancing client well-being.
- Ability to work collaboratively within a team environment.
- Valid Class 5 driver’s license with a current driver’s abstract
- Valid First Aid with CPR
- A clear criminal record check through our online service
- Computer proficiency
- Proof of negative TB test
- Food Safe
- Non-violent crisis prevention and intervention within 3 months of hire.
