Job Overview
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our dynamic healthcare team. Responsibilities
- Deliver comprehensive nursing care to patients in accordance with their individualized care plans, ensuring safety, comfort, and dignity.
- Administer medications and treatments accurately following established protocols and medical orders.
- Monitor patient conditions closely, document observations thoroughly, and communicate changes promptly to the healthcare team.
- Assist with procedures such as phlebotomy (blood draws), wound care, pain management, and other clinical tasks requiring technical skill.
- Support patients with activities of daily living, including mobility assistance, hygiene, nutrition, and emotional support.
- Collaborate with interdisciplinary teams to develop and update care plans tailored to each patient's needs.
- Maintain accurate medical records using electronic health record systems like Epic or Cerner while adhering to privacy regulations.
- Participate in case management activities to coordinate patient services across different providers and settings.
- Engage in dementia care, hospice support, pediatric nursing, senior care, or home care as needed to meet diverse patient requirements.
Experience
- Proven experience working in nursing homes, hospitals, or acute care settings with a strong understanding of medical terminology and clinical procedures.
- Prior experience in dementia care, hospice environments, pediatrics or senior care is highly desirable.
- Familiarity with laboratory procedures such as phlebotomy and laboratory testing processes.
- Knowledge of medication administration protocols and pain management techniques.
- Experience utilizing electronic health records systems like Epic or Cerner for documentation and case management purposes.
- Background in assisted living facilities or home care environments is advantageous.
- Strong communication skills combined with the ability to work effectively within multidisciplinary teams dedicated to delivering exceptional patient-centered care.
