Join the A-Team as a Nurse Practitioner

This is a 3-6 month contract position located in Sudbury, Ontario, with the possibility of a full-time, permanent extension!
Compensation is $70.00/hour to $80.00/hour depending on experience.

Roles and Responsibilities
Clinical Expertise
· Provides holistic assessment of and care for patients and families with a focus on health promotion, disease prevention, health maintenance and rehabilitation.
· Completes health histories and physical examinations using assessment tools as available and applicable to assess strengths and needs.
· Formulates and communicates diagnoses, determines the need for and orders the necessary diagnostic tests as per scope of practice, including labs, X-Rays, ECGs and U/S and interprets the results.
· Orders pharmacotherapy safely, according to most current guidelines when appropriate.
· Develops a plan of care that considers the individual patient's health status, preferences, as well as current best practices.
· Determines the need for and implements health promotion and disease prevention counselling, or refers to existing services in the community.
· Provides patient education to promote attitudinal and behavioural changes that improve health and well-being.
· Assists the patient to gain the knowledge, skills, attitude, and self-awareness necessary to effectively manage their illness.
· Ensures coordinated and comprehensive care through ongoing communication and communication with all members of the patient's care team in order to optimize patient care and ensure optimal delivery of service.
· Maintains ongoing patient documentation, including assessment findings, treatment plans, and progress.
· Prepares statistical records of patient care and related activities as required by the Ministry of Health and Long Term Care, and other agencies.
· Liaises with other service providers, relevant community agencies/programs.
Continuous Professional Development
· Continues to update clinical skills and keep abreast of best practices and relevant resources through continuing education opportunities.
· Demonstrates commitment to professional growth and development for self and colleagues.
Job Requirements
· Ability to travel within Greater Sudbury
· Adhere to Occupational Health and Safety Policies and Procedures
· Adhere to CNPLC policies and procedures
· Ability to work flexible work hours
· Some evening work and home visits are required
· Registered with NPAO
· Understanding of and commitment to continuous quality improvement
· Proficient computer skills: clinical documentation and communication in EMR- Accuro; Microsoft Office
· Ability to integrate new knowledge into an evidence-based practice
· Current Ontario Driver’s License and insurance
· Valid CPR/First Aid
· CPIC required
Education I Experience
· Registered as an RN-EC, Primary Health Care, in good standing with the College of Nurses Ontario
· Valid BCLS
· Masters in a related discipline preferred
· 2 years PHCNP experience preferred
