Crown Ridge Health Care Services Inc operates Westgate Lodge Long-Term Care home, located in Belleville, Ontario. Westgate Lodge will be the home to 160 long-term care residents.

Part-time RAI (every other weekend)

What will you do.
- Maintains the schedule for RAI assessments and completes InterRAI assessments for submission to CIHI as required by MLTC in collaboration with the Director of Nursing.
- Ensures that the RAI process is in accordance to home policies, MOHLTC regulations, College Standards of Practice and interRAI requirements (see CIHI User’s Manual).
- Will complete InterRAI assessments, resident assessment protocols and care plans for all new admissions to the home, according to the time requirements.
- Ensure assessments are correctly coded, develop processes that ensure data accuracy, efficient data collection, assessment completion and data utilization.
- Will ensure that resident plans of care are maintained to reflect current status.
- Monitors the home’s CMI regularly and collaborates with the Director of Nursing to ensure effective documentation and coding.
- Under the direction of the Director of Nursing, generates and reviews InterRAI Output Reports with the management team, front line staff for use in clinical and operational decision-making.
- Participates as a member of the multi-disciplinary team at admission, annual or scheduled care conferences to review and update the plan of care and InterRAI assessment.
- Attends education and training sessions related to InterRAI and position responsibilities.
- Assist with orientation and education of RPN staff.
- Required to function in the RPN role as per the RPN job description in the home as scheduled and as needed, to ensure staffing needs of the home are met.
What will you need.
- Completion of an approved Practical Nursing Program
- Maintains Annual RPN Certification Requirements as per the policy in the home
- Active and in good standing with the College of Nurses of Ontario
- Current CPR and First Aid Certification
- Vulnerable sector check that is less than 6 months old at the time of hire
- TB test steps 1 and 2 proof and 1 step in last 6 months until two step obtained in last year
- Available to work every other weekend
- Experience working with geriatrics, long-term care, or home services is an asset, but not needed
- Proficient with modern software applications and technology platforms; able to quickly learn new systems such as MS Word, Excel, e-mail, internet use, and computerized documentation
- Clear, professional, and effective communicator—both written and verbal.
- Takes initiative to identify issues and develop effective, efficient solutions independently or collaboratively.
- Approachable with a friendly demeanour; able to build strong relationships with residents, families, and coworkers
- Works well as part of a team, contributing positively to group dynamics and shared goals.
- Equally comfortable working independently and taking ownership of tasks and responsibilities.
