This position is designed for an ambitious Registered Practical Nurse (RPN) who is looking to grow beyond traditional bedside nursing into a regional clinical leadership role. Working directly with the Regional Health Care Director, the Clinical Care Coordinator will support multiple retirement residences across Eastern Ontario by strengthening resident care, clinical quality, regulatory compliance, inspection readiness, and standardized clinical systems.
This is not a traditional floor nursing position. You will work alongside the Regional Health Care Director to identify trends, support residence leadership teams, conduct audits, improve clinical processes, and help build consistent standards of care across the organization. A company vehicle is provided.
Key Responsibilities
· Travel between retirement residences throughout Eastern Ontario to provide clinical and operational support.
· Complete resident assessments and participate in comprehensive clinical assessments as required.
· Review, update, and audit resident care plans to ensure they reflect current needs and regulatory requirements.
· Conduct medication audits and monitor medication management practices in collaboration with residence teams and pharmacy partners.
· Complete clinical, documentation, and compliance audits while identifying opportunities for improvement.
· Support RHRA inspection readiness, corrective action plans, and ongoing compliance monitoring.
· Identify trends related to resident care, falls, medications, documentation, infection prevention and quality indicators, and report findings to the Regional Health Care Director.
· Provide education, coaching, and support to nursing staff and residence leadership.
· Assist with development and implementation of standardized policies, clinical tools, and quality improvement initiatives.
· Prepare reports, action plans, and follow-up documentation after site visits.
· Collaborate with interdisciplinary teams to improve resident outcomes across multiple residences.
Qualifications
· Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
· Experience in retirement homes, long-term care, assisted living, or community nursing preferred.
· Demonstrated leadership potential and the ability to coach, influence, and work collaboratively with multiple teams.
· Knowledge of resident assessments, care planning, medication management, clinical documentation, and nursing standards.
· Experience with audits, inspections, or quality improvement initiatives is an asset but not required.
· Excellent organizational, communication, critical thinking, and problem-solving skills.
· Comfortable using Microsoft Office, Microsoft Teams, and electronic clinical documentation systems.
· Valid Ontario driver's licence and willingness to travel extensively throughout Eastern Ontario (company vehicle provided).

Compensation
Full-time, in-person
Salary: Starting at $55,000 annually (commensurate with experience).
