Registered Nurse
Certified Diabetic Educator
The Espanola & Area Family Health Team is an
interdisciplinary group of allied health
professionals promoting improved health of
individuals in and around Espanola by providing
increased access to a range of health care
services.
The successful candidate will have familiarity
with the health issues of the community and will
work collaboratively with other primary care
providers to improve the health of the patients’
accessing the services through the Family Health
Team.
This will be accomplished through active
employee involvement, continuous training, and
development, as well as a commitment to
compliance with policies, procedures, and
standards.

Qualifications and Responsibilities:
Completion of BScN or RN diploma
program with current registration in
good standing with the College of
Nurses of Ontario and national CDE
certification.
Completion of the Diabetes Education
Program Certificate or equivalent.
Demonstrated knowledge of the role of
interdisciplinary health care providers as
part of a collaborative team.
Experience in program development,
implementation, monitoring, and
evaluation.
Experience with individual and group
facilitation of education utilizing various
adult learning principles.
Understanding of community based
primary care prevention programming
and knowledge of chronic disease
management.
Demonstrated understanding and
application of the vision, mission and
values of the Family Health Team
initiative.
Good organizational skills and time
management skills. Demonstrated
ability to manage multiple tasks,
prioritize workload.
Attention to detail, excellent
communication skills and strong
interpersonal skills.
Ability to provide regular attendance.
Knowledge of Consent and Release of
Information legislation
Bilingualism (English and French) is an
asset
Benefits:
- This position offers a competitive
compensation package including pay in
lieu of vacation and benefits. The wage is
$36.696 – $41.449 per hour based on
experience
