JOB SUMMARY
Reporting to the director, the incumbent will be responsible for providing support to participants in assisting them in achieving goals that will lead to successful community living. The incumbent will be responsible for assessing individual’s level of functioning and mental health, developing, documenting, and implementing of individual rehabilitation plans and individual intake interviews. The incumbent will be responsible for all aspects of assisting participants with housing, community integration and employment. This is a bilingual position in both official languages – English and French (Not required but preferred). This position provides Emergency On-Call rotation. The incumbent is required to workdays, evenings and weekends, 7 days a week on a rotation schedule. Daily schedule may vary according to program needs.
QUALIFICATIONS AND ABILITIES
- Social Worker.
- 2-5 years’ experience working with adults with mental health issues.
- Strong written and verbal communications skills in English and French (Not required but preferred)
- Satisfactory Employment record.
- Ability to function in a demanding and stressful environment.
- Demonstrated ability to work with minimal supervision.
- Provide a current and satisfactory Criminal Record Check and Adult and Child Abuse Registry Check.
- Complete WHMIS training and understand the safety precautions and emergency procedures of potential hazards.
CASE MANAGEMENT
- Assessment of individual functioning, identifying barriers to community living rehabilitation goal setting, and individual self-evaluations are included in individual assessments at lead practice standard complete.
- When intake and assessments are completed, approved documentation formats are used and documented.
- A complete review of individual status is done a minimum of every 3 months.
- Written rehabilitation and individualized plans, with objectives based on initial individual assessment, are completed in collaboration with the individual in the approved format and filed within five (5) days of completion of the assessment period.
- Ensure rehabilitation plans will be reviewed, revised, and implemented within the stated program time frames and/or minimally on a monthly basis.
- Ensure intervention strategies are incorporated into the rehabilitation plan.
- Professional support and counselling is provided in accordance with individual's assessed needs and documented in the individual’s progress notes.
- Individuals demonstrate increased participation in the community and ability to achieve rehabilitation goals.
- Crisis situations are assessed and addressed immediately. Relevant information is communicated to other staff prior to end of shift for follow-up and information is documented in the progress notes.
- Collaboration with family members and external professionals involved with individuals, in accordance with person’s requests, policy and procedure and legislation concerning significant developments in progress is evident.
- Facilitate all matters related to closing individual file and transition from employment services planning.
- Assist with finding housing as needed.
- Annual review with all relevant collaterals.
- Documentation at a high level.
- PROGRAM RESPONSIBILITIES
- Participate in team meetings.
- Maintain individual files, including continuous updates to Sara Riel Inc. database.
- Maintain program statistics.
- Assist Manager with program assessment and development when requested.
- Investigate and complete incident reports when required.
- Develop and facilitate workshops.
- Confidentiality as per related legislation and policy.
- OUTREACH AND COMMUNITY AWARENESS
- Develop and maintain on-going professional relationships with community resources.
- Market program services in the community.
- ADMINISTRATIVE/OTHER DUTIES
- Provide all services in accordance with Sara Riel policies and procedures.
- Work with Sara Riel Staff, counsellors to hold special events for individuals and staff.
- Adhere to and maintain SPA statistics and employment expectations.
- Represent self and agency in professional manner at all times.
- Other duties as assigned.
EMERGENCY ON-CALL SERVICES
- Mental Health Counsellors will provide after hours On-Call support.
- On-Call will respond to individuals and any potential after-hours staff for support as per Sara Riel Inc. On-Call guidelines.
- Staff will ensure On-Call data and activities are updated with current information.
- Ensure all On-Call equipment is always in operational.
EVENT PARTICIPATION:
- Participate in initiatives for Sara Riel functions/events at least once per year.
- Support could in include planning, development, coordination and general participation with events.
- Company events may take place both during and outside regular working hours. Employees participating in an event outside of working hours will be permitted to flex their time.
Pay: $24.71-$27.00 per hour
