POSITION DESCRIPTION
Position: Rural Link Worker
Position Type: Full-Time (1.0 FTE), contract ending June 30, 2027
Reports to: Seniors System Navigator Team Lead
Position Summary:
Provide service connection to older adults in Lethbridge County, Cardston and Cardston County who identify as needing resource connection or caregiver support.
Client Support & Programming:
- Complete needs assessments and make referrals to rural community resources and those that require transportation into Lethbridge or other communities in the catchment area;
- Provide assistance in filling out forms and applications;
- Provide general information concerning government and non-government programs, benefits and services to seniors including relevant legislation and regulations (Personal Directive Act, Social Housing Act, Protection for Persons in Care Act etc.).
- Attend appointments or programs with older adults to overcome barriers, where appropriate;
- Collaborate with other agencies and professionals in the community for the benefit of the client;
- Implement support groups in rural areas.
Community Partnerships & Program Development:
- Develop partnerships and referral pathways with rural agencies and senior serving stakeholders;
- Actively liaise with rural senior serving organizations, health organizations, community groups and peer support groups to explore and address issues;
- Collaborate with rural senior serving agencies to implement SCSP programming and ensure there is no duplication of services.
Administration:
- Support quarterly collection of data required by funders;
- Responsible for all data entry, case notes and reports pertaining to position;
- Maintain private and confidential information appropriately within a home office;
- Identify program improvement opportunities
- Keep up to date on resources and information pertaining to seniors.
- Other duties as assigned.
Education & Qualifications:
- Bachelor of Social Work (BSW), Recreational Therapy, Public Health, or related education;
- Experience working with the senior population and/or relevant case management experience;
- Ability to analyze, apply critical thinking and problem-solving skills while working in a fast-paced environment with the ability to balance multiple tasks/requests, as well as prioritize presenting needs;
- Able to work independently but collaborate within internal and external multidisciplinary teams;
- A broad knowledge of personal and social issues affecting seniors and caregivers;
- Adequate experience and skill with personal computers and office productivity software
- Valid driver’s license and $2,000,000 liability insurance: vehicle required
- Current Criminal Record Check (within 90 days of employment)
