PURPOSE: Reporting to the Sul’hween Program and Nursing Manager, the Personal Care Worker is responsible to provide personal care assistance, homemaking and support to program participants and their families/caregivers to promote and maximize independence within the home and community setting, including assistance with activities of daily living and delegated tasks as directed and in accordance with established care plans, and provide administrative support to the program. This is a casual on-call unionized position that works primarily off-site with extensive local travel and time on-site as required.
COMPENSATION AND BENEFITS: Depending on experience, the position starts at $25.67 - $27.20 per hour ($46,716.52 - $49,509.08 annually).
RESPONSIBILITIES:
Provide Personal Care Assistance, Homemaking and Support to Program Participants and their Families/Caregivers:
- Assist with medication delivery as per individual's care plan
- Provide personal care services program participants according to their care plan
- Assist program participants in bathing and personal hygiene tasks
- Prepare meals for clients as per their dietary needs
- Provide general home making services according to the client’s care plan which may include but not limited to laundry, dusting, general cleaning, vacuuming, washing floors and kitchen as required
- Assist program participants to engage in social activities when required
- Support participation in program activities and events
- Report any client health concerns or issues immediately to the Sul’hween Program and Nursing Manager
- Provide occasional transportation support and accompany program participants to medical appointments and grocery shopping when needed
- Develop and maintain effective relationships with program participants and their families, and members of the interdisciplinary health care team
- Ensure proper personal protective equipment (PPE) protocols are taken when providing care
- Perform duties in a professional manner ensuring optimal care is provided and duties are performed to a high degree of standard.
Provide Administrative Support to the Program:
- Attend Elders Program team meetings as required
- Be aware of internal/external resources, such as Ts’ewulhtun Health Center programs and services, physicians, community partners and services, and communicate to program participants or refer to appropriate member of the health care team as appropriate
- Consult with the Elders Nurses, Personal Care Worker Team Leader and/or the Sul’hween Program and Nursing Manager regarding clients and services throughout scheduled shift as needed
- Ensure accurate and detailed record management of all encounters with program participants using an electronic medical record system (EMR) and report any health concerns to the Elders Nurses and/or the Sul’hween Program and Nursing Manager immediately
- Promote safety, check in before and after visits, and report any safety concerns to the Personal Care Worker Team Leader and/or Sul’hween Program and Nursing Manager
- Other related duties as directed.
EDUCATION AND EXPERIENCE: (please attach all required documents)
- Health Care Assistant Program certification, or equivalent
- 1 year health care assistant/personal care worker experience
- Registration in good standing with BC Care Aide & Community Health Worker Registry
- Experience working in a First Nations community is preferred
- Experience or willingness to learn about culture, customs, and traditions of Cowichan people
- Certification in First Aid level 1 / CPR C/ FoodSafe
- BC Drivers’ License and reliable vehicle
- Criminal Record Check with vulnerable sector check
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of Cowichan culture, customs, and traditions
- Knowledge of health issues that face First Nations peoples
- Knowledge of proper cleaning procedures and products
- Well-developed written, oral, interpersonal and organizational skills
- Able to foster and build relationships within a First Nations population
- Able to work a flexible schedule, including mornings and evenings as needed, between 7:30 AM – 8:30 PM
- Time management with the ability to multitask different tasks simultaneously
- Physically able to perform duties of the job which require stamina – able to perform repetitive tasks, lift, squat, bend and other physical duties required for housekeeping as needed
- Able to operate related equipment such as mechanical tub
