Position Overview We are seeking a compassionate and attentive Personal Support Worker to conduct a weekly 2-hour wellness check-in visit for an elderly male client in the early stages of cognitive decline. The primary purpose of this visit is to provide the client's family with peace of mind through regular observation, light support, and companionship, with particular attention to recent wandering behaviour.
Client Background The client is an elderly man experiencing early-stage cognitive decline. He has recently shown signs of wandering behaviour (leaving the home unattended or becoming disoriented about location or time). He is able to manage daily activities independently. The family is requesting a trusted, familiar presence to check in on him weekly and flag any changes or concerns.

Key Responsibilities
Safety and wellness check Assess the client's general physical and mental state upon arrival. Check that the home environment is safe and free of hazards (unlocked doors, stove left on, tripping hazards). Confirm the client knows the day, date, and general orientation to time and place, noting any changes since the last visit.
Companionship and engagement Spend time in conversation or a shared activity of the client's choosing (reading, puzzles, listening to music, going through photos) to provide social stimulation and reduce agitation or restlessness.
Light assistance with daily living Support with light tasks as needed, such as preparing a light snack or beverage, tidying a small area, reminding the client of medication schedules (not administering medication unless certified and authorized to do so), and helping with grooming reminders if appropriate.
Observation and reporting Note any changes in mood, memory, physical health, appetite, or behaviour compared to previous visits. Document the visit using a mobile app (arrival/departure time, observations, any incidents) ). Communicate any concerns to the family promptly, especially anything related to wandering, safety incidents, or a noticeable decline in condition.
Required Qualifications Completion of a recognized Personal Support Worker (PSW) certificate program Current CPR and First Aid certification Experience working with clients with early-stage dementia or cognitive decline preferred Vulnerable sector police check (current within the last 12 months) Reliable transportation to the client's home
