Sinai Health is seeking a detail-oriented and compassionate Patient Liaison to join our dedicated team at the High-Risk Pregnancy Clinic. The Patient Liaison plays a key role in managing the referral process, ensuring timely and complete intake of patient information, and facilitating smooth communication between referring providers, patients, and our care team.
This role works closely with medical secretaries, triage nurses, and a wide range of specialists to provide seamless care coordination for patients with complex pregnancy needs.

The Special Pregnancy Program (SPP) is comprised of the Ontario Fetal Centre (OFC) and the Medical Disorders of Pregnancy (MDP) clinics in the Women’s and Infants’ Program at Mount Sinai Hospital.
The OFC, based at Mount Sinai Hospital, and in partnership with The Hospital for Sick Children, is the first in Canada, and one of only a few centres worldwide, to offer a fully comprehensive range of fetal therapy.
The MDP program provides specialized high-risk pregnancy care for patients with pregnancy-related or pre-existing medical conditions.

In this role you will:
- Receive and review referrals from provider offices
- Ensure referral packages are complete, including all required supporting documentation
- Follow up with referring providers to collect missing information or clarify clinical details
- Contact patients to schedule initial and follow-up appointments
- Provide patients with appointment details, instructions, and information about specialty services
- Manage appointment changes, cancellations, and re-bookings in a timely and patient-centered manner
- Collaborate with triage nurses and medical secretaries to prioritize referrals and facilitate urgent bookings when necessary
- Coordinate appointments across multiple specialties (e.g., cardiology, endocrinology, genetics) relevant to maternal and fetal care
- Maintain accurate and up-to-date records of referral status and patient interactions
- Track and monitor incoming referrals to ensure timely processing
- Perform other duties consistent with the job classification as required
