Overview
We are seeking a dedicated Primary Health Care Nurse Practitioner under the primary care program to join our healthcare team. Duties
- Conduct thorough patient assessments and develop individualized care plans.
- Provide medical diagnoses and prescribe appropriate treatments in accordance with state licensure.
- Manage case coordination for patients with complex health needs,
- Collaborate with multidisciplinary teams utilizing electronic health records systems such as Accuro
- Offer patient education on health maintenance, medication use, and disease prevention strategies.
- Assist in managing laboratory tests and interpreting results to inform clinical decisions.
- Document all patient interactions accurately in medical records while adhering to confidentiality standards.
Skills
- Strong knowledge of medical terminology, medication administration, and case management principles.
- Experience with electronic health record systems such as Accuro is preferred.
- Excellent patient care skills with a compassionate approach to diverse patient needs.
- Ability to perform phlebotomy and manage dementia or other complex care cases effectively.
- Skilled in pain management strategies and laboratory procedures relevant to primary care practice.
- Effective communicator capable of collaborating within multidisciplinary healthcare teams in medical offices or community settings. Pay: $38.97-$113.09 per hour
