We are seeking a compassionate and dedicated Nurse Practitioner (NP) to join our team for a 6-month contract, with the opportunity to extend. In this role, you will lead a distinct Primary Care program that operates alongside an existing Addiction Medicine program.
While our site hosts specialized addiction services, your role is focused entirely on the comprehensive primary care needs of our patients—addressing their general health, chronic disease management, and preventative care needs independently of their addiction treatment journey. This position offers a unique blend of flexibility, allowing you to make a meaningful impact through both remote telehealth consultations and in-person clinical care across our Ontario-based sites.
Position Overview
- Role: Nurse Practitioner (Primary Care)
- Employment Type: Part-Time | 6-Month Contract (with the possibility of extension)
- Compensation: $80 - $110 per hour (commensurate with experience)
- Work Model: Hybrid (Remote telehealth sessions + required travel to clinic sites)
- Location: Various locations across Ontario
Key Responsibilities
- Primary Care Lead: Provide comprehensive primary care for patients, focusing on chronic disease management, preventative screenings, and acute episodic care.
- Independent Practice: Operate as a primary care provider within a dedicated program, ensuring patients receive high-quality medical oversight for their general health needs.
- Collaborative Environment: While the primary care program is separate from the addiction program, you will work within the same facility, allowing for seamless, patient-centered communication and holistic support.
- Hybrid Workflow: Conduct remote telehealth visits and provide regular, on-site clinical care at our physical clinic locations.
- Documentation: Maintain accurate, thorough, and timely electronic medical records (EMR) in compliance with CNO and provincial standards.
Important Note on Travel
Please note: This position requires regular travel to our physical clinic locations across Ontario. While a portion of the workload is remote/telehealth, the successful candidate must be able to commute to our clinics to provide hands-on care. Candidates are expected to have a reliable method of transportation and the willingness to travel within the designated service regions as part of their weekly schedule.
Qualifications
- Licensure: Current, unrestricted registration as a Nurse Practitioner (Primary Health Care) with the College of Nurses of Ontario (CNO).
- Experience: Strong background in Primary Care with the ability to manage complex medical histories. Experience working with diverse, vulnerable populations is highly valued.
- Professionalism: Ability to work effectively and maintain clear professional boundaries while operating alongside specialized addiction programs.
- Flexibility: Ability to work effectively in a hybrid environment, balancing independent remote work with active on-site clinical participation.
- Requirements: Valid Ontario driver’s license and the ability to travel to clinic sites as required.
