Canadian Mental Health Association – Thunder Bay Branch is a non-profit organization committed to supporting individual recovery and promoting mental wellness within families and communities throughout northwestern Ontario.
What is the job!
In collaboration with the First Place Clinic multidisciplinary clinical team and other community partners, the Nurse Care Coordinator will provide assessment, planning and treatment services to adolescents and adults who are experiencing a first episode of psychosis within the Northwest Region, which includes travel to First Nations and surrounding communities.
The Nurse Care Coordinator will integrate medical and recovery models of care in their practice, and engage in therapeutic partnerships with clients, family members, colleagues, and community services.
What will you do day to day?
- In collaboration with the multidisciplinary team, they will conduct initial screening on new referrals, participate in initial assessments, and establish therapeutic partnerships with clients and their families for the purpose of case management, medication management, treatment and wellness planning, all while following the recovery values of client centered care and shared decision making
- Integrating medical and rehabilitation approaches of practice into service delivery through the coordinated specialty care model (Navigate).
- Demonstrating ability to work as a team player within a multidisciplinary team model of care through collaborative practice, respectful approaches to all disciplines/backgrounds represented and openness to input and feedback
- Assigned nursing functions within scope of practice.
- Providing psycho-education, crisis prevention and intervention, advocacy and referral services on behalf of clients as needed
- Developing and facilitating group programs for clients and families
- Administrative responsibilities including attending and participating in meetings as required, accurate and timely completion of all documentation requirements and statistics
- Maintaining up to date knowledge and skills in early psychosis intervention through self-guided learning, training opportunities, supervision and performance appraisals
- Developing and delivering educational training programs, internally and externally to students, professionals, and the public/community
- Actively contributing to research, program evaluation and program development initiatives as appropriate to role
- Communicating regularly with circle of care
- Adhering to and promoting CMHA’s values and Code of Ethics Policy
- Complying with CMHA’s Health and Safety Policies
- Completing other duties as assigned
What do you bring to the job?
- Diploma or Degree in Nursing
- Current Certificate from the College of Nurses in Ontario
- Eligible to practice as a Registered Nurse or Registered Practical Nurse in Ontario
- Understanding of first episode psychosis and best practice approaches
- Strong communication and interpersonal skills; ability to establish and maintain positive relationships with all staff, clients, hospital and community partners
- Experience working with adolescents/young adults who may also have substance use issues is an asset
- Training, knowledge and experience working with clients who have psychiatric illness, with particular emphasis on psychosis and concurrent disorders is an asset
- Knowledge of developmental issues and needs of young adults, their families, caregivers, and a sensitivity to the needs of First Nations
- Good communication, engagement, problem-solving and conflict resolution skills
- Knowledge, skills, and experience, in integrating medical and recovery models of care with clients
- Ability to work collaboratively with, and knowledge of community resources, to support clients, families, caregivers, team members, hospital and community resources
- Computer skills using Microsoft Office products and Client Management Systems
- Knowledge and comprehension of the Mental Health Act, Healthcare Consent Act, Substitute Decisions Act and Privacy legislation
- Strong work ethic and empathetic to needs of clients with mental health issues
- Ability to speak French is considered an asset
- Police Vulnerable Sector Clearance
- Ability to travel within the surrounding community
- Valid Class “G” driver’s license, reliable vehicle and $2,000,000 vehicle liability insurance
- Legally eligible to work in Canada
