Job purpose:
The Community Resource Facilitator, facilitating our Individualized Senior Support Program (ISSP) in partnership with Toronto Family Services Association and Seaton House, City of Toronto, will provide assistance and access services for seniors with mental health problems to remain housed in the community.
Responsibilities include, but not limited to, the following:
· Conduct comprehensive assessments using OCAN to identify the unique needs, strengths, and goals of senior participants
· Develop personalized support plans in collaboration with seniors and their support networks to address mental health, addiction, and disability-related barriers
· Connect seniors with relevant community resources, including healthcare providers, social services, housing supports, recreational activities, and employment opportunities
· Advocate on behalf of seniors to ensure their voices are heard and their rights are respected within the community and service systems
· Empower seniors to actively participate in decision-making processes that affect their lives, fostering self-advocacy and independence
· Facilitate educational workshops and training sessions on topics such as mental health management, addiction recovery, and life skills enhancement
· Provide information and guidance to seniors and their caregivers to promote health literacy and informed decision-making
· Maintain accurate and confidential records of participant interactions, progress, and outcomes
· Prepare regular reports and updates for program managers and funding agencies, highlighting achievements, challenges, and areas for improvement
· Maintain up-to-date knowledge of local resources and services, continually expanding the network of available supports
· Collaborate with internal program teams and external partners/client support workers to ensure seamless service delivery and holistic support for seniors
· Build and maintain positive relationships with community stakeholders, advocating for the needs of seniors and promoting program visibility
· Attend monthly meetings with Family Services Toronto (FST) and monthly partnership meetings with other agencies
· Carry out administrative duties such as invoicing, keeping track of client budgets, etc.
· Perform other tasks and duties as required and assigned
Qualifications:
- Post-secondary education in social services, degree in social work, college diploma in social services with a specialty in mental health or equivalent experience
- Previous experience working in a community agency or hospital environment as a support worker or social worker for 2-3 years minimum
- A strong desire to contribute to a community based organization and the improvement of the quality of life for a marginalized community
- Demonstrated assessment, coordination and crisis intervention and counselling skills
- Excellent written and oral communication skills
- Strong interpersonal skills with a high level of social and emotional intelligence
- Great Life skills coaching and crisis management skills
- Good understanding of issues faced by people with mental health and aging
- Good understanding of the recovery model and anti-oppression framework, and a familiarity with issues affecting ethno-racial and lesbian, gay, bisexual, transgendered and queer communities
- Current Standard First Aid/CPR&AED training an asset
- Ability to work with minimal supervision and in a team environment
- Approachable and sensitive to the needs of others
- Fluency in languages other than English is an asset
Working conditions:
- Primarily work indoors
- Under normal circumstances, work in close proximity with others with near-constant interaction with clients and/or residents and fellow staff
Physical requirements
- Use of hands – typing to input client information into database and case management software
- Use of PPE – face mask, face shield, gloves, gowns, etc. as needed
