Get Well Family Health Team is a collaborative, patient-centred primary care team serving North York area. Our team includes physicians, nurse practitioners, RNs, RPNs, social workers, dietitians, and other integrated health professionals committed to providing comprehensive, high-quality care.
Role Description: We are seeking a motivated and compassionate Registered Nurse (RN) to join as nurse team lead to supervise our nurses and clinical services. The RN will provide clinical care, support chronic disease management, and assist with health promotion and patient education as well as provide supervision to nurses and interdisciplinary health providers. This role works closely with physicians, NPs, and other team members to ensure coordinated, patient-focused care.
Key Responsibilities:
- Perform nursing assessments and interventions within RN scope of practice.
- Provide leadership in a supervisory, team lead role.
- Administer medications, injections, immunizations, and wound care.
- Support chronic disease management programs (e.g., diabetes, hypertension).
- Provide health education and counseling to patients and families.
- Collaborate with the interprofessional team to optimize patient care.
- Provide clinical intake, triaging, and system navigation services to patients.
- Liaise with healthcare providers and facilitate referrals within the Family Health Team and to secondary/tertiary care.
- Ensure practice aligns with professional standards, ethical guidelines, and organizational policies.
- Engage in continuing education and reflective practice to enhance patient care quality.
- Perform cross-functional duties and other assigned tasks as needed.
- Promote positive public relations and support a family-centered care environment.
- Assist with clinic administration, including clinical coaching, inventory management, and maintaining supplies, medications, vaccines, and PPE.
- Documentation for medical, statistical, and administrative purposes.
- Maintain accurate and confidential patient records and documentation.
- Participate in group programs, quality improvement initiatives, and team meetings.
- Take on additional initiatives that contribute to the growth of the health team.
Qualifications:
- Bachelor of Science in Nursing from an accredited institution.
- Registered and in good standing with the College of Nurses of Ontario.
- Valid CPR HCP certification.
- Minimum 2 years’ experience in community or primary care, including chronic disease management and health teaching.
- Strong interpersonal, organizational, and multitasking skills; detail-oriented and team-focused.
- Excellent telephone and customer service skills.
- Proficient in Microsoft Office; EMR experience (e.g., Oscar EMR) is an asset.
- Flexible availability, including some evenings and Saturdays.
- Able to work at multiple locations within North York
- Fluent in English, additional languages reflecting the community (e.g., Cantonese, Mandarin, Farsi, Korean and Tagalog) are an asset.
- Valid vulnerable sector police record check completed within the past year.
