About Senior Support North Vancouver Island
Senior Support North Vancouver Island (SSNVI) is a community-based non-profit organization supporting older adults across the Campbell River and Comox Valley regions. Through programs, services, volunteers, and community partnerships, SSNVI helps older adults remain connected, independent, and engaged in their communities while reducing isolation and promoting healthy aging.
Position Summary
The Seniors Community Connector supports older adults in the Campbell River region to age safely and well in the place of their choosing by providing individualized Social Prescribing support and strengthening community connections.
Social Prescribing is a person-centred approach that helps older adults improve their health and well-being by connecting them with community-based resources, activities, relationships, and supports that reflect their interests, strengths, goals, and what matters most to them. It recognizes that social connection, meaningful activity, and a sense of belonging are important parts of healthy aging.
The Seniors Community Connector works directly with older adults to reduce social isolation, increase participation in community life, and support access to opportunities that promote connection, independence, and quality of life. This includes building trusting relationships, exploring personal goals, identifying barriers, and connecting individuals with programs, services, and resources that support their well-being.
The Community Connector provides relationship-based support and resource navigation rather than clinical or case management services. The role works collaboratively with existing health and social service systems to strengthen connections, improve access to community resources, and support older adults in achieving their personal goals.
In addition to working directly with older adults, the Community Connector plays an important role in strengthening the broader community network that supports healthy aging. The role builds relationships with healthcare providers, community organizations, Indigenous partners, local governments, and other stakeholders to improve collaboration, identify gaps and emerging needs, and support more connected, inclusive, and age-friendly communities.
Recognizing the unique realities of rural and regional communities, the Community Connector understands that building sustainable connections, partnerships, and opportunities requires time, relationship-building, collaboration, and creativity.
Key Responsibilities
1. Supporting Older Adults Through Social Prescribing (50% of role)
- Build trusting, strengths-based relationships with older adults.
- Meet with participants to understand their interests, strengths, goals, preferences, and what matters most to them.
- Use meaningful conversations, active listening, and motivational interviewing approaches to support goal setting and exploration of opportunities.
- Co-develop individualized connection plans that support social connection, participation, and well-being.
- Connect older adults with community programs, services, activities, and resources that align with their goals and interests.
- Support older adults in accessing meaningful opportunities for recreation, social connection, volunteering, learning, cultural participation, and community involvement.
- Provide resource navigation, encouragement, advocacy, and follow-up support.
- Identify and help address barriers that may prevent older adults from participating in community life.
- Support older adults to build confidence and independence in accessing community opportunities and supports.
- Recognize when additional supports may be needed and assist with connections to appropriate services.
- Maintain professional boundaries, confidentiality, and ethical practice.
The Community Connector recognizes that older adults may experience complex social, health, financial, mobility, and accessibility barriers. While the role supports individuals in navigating available resources and connections, it does not replace clinical, housing, financial, or case management services.
2. Community Capacity Building & Collective Impact (40% of role)
- Build and maintain collaborative relationships with healthcare providers, community organizations, Indigenous partners, municipalities, and other stakeholders.
- Conduct outreach and relationship-building with organizations, groups, and partners that support older adults.
- Develop and maintain an up-to-date understanding of community assets, programs, services, resources, and referral pathways available to older adults.
- Strengthen referral pathways between healthcare providers, community organizations, and social supports.
- Identify community needs, service gaps, and barriers affecting older adults.
- Participate in community tables, working groups, networking opportunities, and collaborative initiatives that strengthen healthy aging.
- Support improved coordination between services and increase awareness of available resources.
- Contribute community insights and participant experiences to support program development, service improvements, and systems change.
- Participate in Communities of Practice and other learning opportunities to strengthen Social Prescribing practice.
- Support outreach, education, and awareness of Social Prescribing within the community.
- Build community capacity by developing relationships, strengthening partnerships, and supporting the creation of sustainable pathways to connection and participation for older adults.
3. Program Administration & Evaluation (10% of role)
- Maintain accurate and timely documentation of participant interactions, goals, referrals, and follow-up activities.
- Maintain accurate and confidential electronic records in accordance with organizational policies, applicable privacy legislation, and ethical standards.
- Collect meaningful program data and contribute to evaluation activities that demonstrate participant outcomes, community connections, partnership development, and program impact.
- Support reporting requirements and continuous quality improvement.
- Participate in team meetings, training, and professional development.

Qualifications
- Diploma or degree in Social Services, Recreation Therapy, Gerontology, Community Development, Human Services, or a related field; or an equivalent combination of education and relevant experience.
- Minimum two (2) years of experience supporting older adults and/or individuals experiencing barriers to community participation.
- Knowledge of healthy aging, social connection, community engagement, and the factors that influence older adults’ well-being.
- Experience working with diverse populations from different cultural and socio-economic backgrounds.
- Knowledge of community resources, programs, activities, and services available to older adults in the Campbell River region.
- Experience in community development, recreation programming, volunteer coordination, social service navigation, or working collaboratively with healthcare and community organizations is an asset.

- Ability to work independently in a community-based role, including a variety of community settings, while maintaining strong collaboration with team members, participants, and community partners.
- Demonstrated ability to exercise sound judgment, manage competing priorities, and seek support when needed.
- Strong computer skills, including proficiency with Microsoft Excel for data tracking and reporting, Microsoft Office applications, and virtual meeting platforms.
- Demonstrated commitment to confidentiality and ethical practice.
- Ability to recruit and support volunteers is an asset.
Conditions of Employment
- Valid BC Driver’s Licence and access to a reliable vehicle with appropriate insurance.
- Ability to travel and work independently in the community throughout the Campbell River region to meet with participants, attend community meetings, conduct outreach, and build community partnerships.
- Successful completion of a BC Ministry of Justice Criminal Record Check as a condition of employment.
- Ability to participate in occasional community meetings, events, or activities outside of regular work hours with advance notice.
- Mileage reimbursement is provided in accordance with organizational policy.
