Yaletown House is a non profit care facility for seniors with complex care needs and is located in Vancouver, BC. We provide quality health care and services to 127 senior residents in a safe, supportive and home like environment.
Under the direction of the Director Resident Services or designate, and working collaboratively with the interdisciplinary team, the registered nurse provides individualized nursing care to residents by promoting quality of life consistent with current Nursing Standards of Professional Practice under the College of Registered Nurses Association of BC and the philosophy of care relating to policies/procedures of Yaletown House Society.
Performs other related duties as providing work direction and guidance to the Licensed Practical Nurses and Care Aides in their application of resident care, assigning tasks and demonstrating work routines.
PRIMARY RESPONSIBILITIES:
- According to BCCNM Standards of Professional Practice, provides direct nursing care to residents, including: (1) assisting residents to maintain, regain or maximize physical and physiological functioning, (2) assisting residents to meet their psychological and emotional needs, (3) supporting residents in the event of a sudden or unpredictable change in status, (4) communicating relevant information about resident’s status to significant others.
- Assesses, plans, implements and evaluates nursing care of residents, through diagnosing actual or potential problems and strengths; planning interventions; performing the planned interventions and evaluating the outcomes.
- Documents relevant observations including resident and family teaching and evaluation of nursing care.
- Manages individual resident care according to the needs of the resident. Provides work direction to direct care staff in their application of basic care such as lifts and transfers, treatments and social programs. Holds regular direct care team meetings and coaches and consults with care giver staff. Reassigns duties as necessary.
- Collects information from a variety of sources, in collaboration with members of the health care team, using skills of observation, communication and physical assessment to enable the provision of appropriate nursing care.
- Communicates, participates and consults with members of the health care team about resident care; participates in multidisciplinary conferences; explains plan of care to residents and their family.
- Acts as an advocate to protect and promote the residents’ rights to autonomy, respect, privacy, dignity and access to information.
- Participates as an active member on nursing and facility committees and councils for improvement of resident care and/or work environment.
- Participates in orientation of direct care staff and students and is responsible for collegially sharing information and demonstrating/providing guidance on clinical tasks.
- Participates, at the direction of the Director Resident Care and or the Clinical Practice Manager, in program evaluation and continuous quality improvement activities within the facility.
- Identifies self-learning needs and attends educational programs to maintain and enhance clinical competency.
- Participates in the evaluation of resident care programs, techniques, standards and procedures by providing feedback about effectiveness and resident care outcomes.
- Maintains a safe environment for resident, families, visitors and staff by adhering to established safety and emergency practices and procedures. Completes accident and incident reports.
- Performs other related duties as required.
QUALIFICATIONS:
Education, Training and Experience
- Graduate of an approved school of nursing.
- Current practicing registration with the British Columbia College of Nurses & Midwives (BCCNM).
- Valid First Aid certificate and CPR certification.
- A minimum of four years of geriatric nursing experience.
Skills and Abilities
- Ability to communicate effectively both verbally and in writing.
- Knowledge of clinical geriatric care needs and knowledge of the Nursing Process in a complex care environment.
- Ability to demonstrate and/or provide guidance to the care provided by Licensed Practical Nurses and Care Aides in a team environment.

- Ability to carry out the physical duties of the position which include lifting, transferring and moving residents safely.

Pay: $41.42-$55.91 per hour
