We are seeking an experienced and compassionate Care Manager to join our team. We are looking for a strong healthcare leader who is passionate about resident-centered care, team development, and operational excellence.
The successful candidate will be a Licensed Practical Nurse (LPN) or a Health Care Aide (HCA), with leadership experience in a Long Term Care setting. This role provides clinical and operational leadership while supporting care teams, coordinating staffing needs, and ensuring the delivery of safe, high-quality resident care.
The ideal candidate will have experience working in a unionized healthcare environment and demonstrate strong knowledge of employee relations, collective agreements, scheduling, and team leadership.
Key Responsibilities:
- Provide leadership, coaching, and support to care teams to promote safe, compassionate, and person-centred resident care.
- Oversee daily care operations and collaborate with interdisciplinary teams to ensure resident needs are met.
- Support the development, implementation, and evaluation of individualized resident care plans.
- Promote compliance with healthcare standards, regulatory requirements, and Select Senior Services policies and procedures.
- Provide Mentorship and support to HCAs, and other care team members.
- Support recruitment, onboarding, orientation, training, and retention initiatives.
- Assist with employee relations matters, including attendance management, performance discussions, investigations, and conflict resolution.
- Collaborate with Human Resources and leadership teams to support effective employee relations.
- Interpret and apply collective agreements, policies, and procedures within a unionized environment.
- Promote positive labour relations and maintain respectful communication with unionized employees.
- Coordinate staffing needs, including scheduling adjustments, shift coverage, call-outs, and replacement planning.
- Work closely with the scheduler to ensure appropriate staffing levels and operational efficiency.
- Participate in quality improvement initiatives, audits, care conferences, and operational meetings.
- Promote a positive workplace culture focused on teamwork, accountability, resident safety, and excellence in care.
Qualifications:
- For LPN applicants, a current and active LPN license in good standing.
- For HCA applicants, a current and active HCA Registry number in good standing.
- Previous leadership or supervisory experience in Long Term Care, Assisted Living, or a healthcare environment.
- Experience managing healthcare teams in a unionized workplace.
- Strong understanding of collective agreements, labour relations, and employee relations practices.
- Experience with staff scheduling, workforce coordination, and operational planning.
- Knowledge of resident care standards, healthcare regulations, and quality improvement practices.
- Excellent communication, organizational, and problem-solving skills.
- Ability to manage multiple priorities in a fast-paced healthcare environment.

Pay: $75,000.00-$90,000.00 per year
