The Personal Support Worker (PSW) supports the mission, vision, and values of Jessica’s House Hospice (JHH). They will work collaboratively in the interdisciplinary team and will liaise with all other healthcare providers to develop and maintain a client-centered plan of care. The PSW is expected to be a positive role model for all staff, volunteers, students, families, visitors, guests, and the community.
The PSW must possess a certificate of completion from an approved learning institution. To perform the job successfully, the PSW must have strong organizational and prioritization skills as well as demonstrated teamwork skills. The PSW must possess a clear understanding of ethics, especially as it relates to hospice palliative care.
The PSW will collect information from a variety of sources using skills of observation and communication and will relay information within an appropriate time frame to the registered staff. The PSW acts as an advocate to protect and promote the residents’ rights to autonomy, respect, privacy, dignity, and access to information. Under the direction of registered staff, the PSW assumes the responsibility for providing direct care to the residents and documenting such care according to the care plan.
POSITION TYPE
The role of PSW may be a part time position and is employed by JHH, the operating company. The position is in Exeter, ON.
RESPONSIBILITIES
Assume responsibility for all aspects of resident care and wellbeing.
Act as an advocate to protect and promote the resident and family rights of autonomy, respect, privacy, dignity, and access to information.
Recognize resident health status changes and report to registered staff in a timely manner.
Monitor residents and their families and report findings to other team members as required.
Work collaboratively with the volunteers to fulfill all residents’ personal and instrumental care needs.
Administer medication and treatments as required.
Document accurately and in a timely manner according to JHH policies.
Monitor and maintain a safe and healthy environment.
Confer with residents and families regarding their satisfaction with their care.
Provide emotional support to residents and families as required.
QUALIFICATIONS
PSW certificate of completion from an approved learning institution
Minimum of two (2) years’ experience working with individuals and their families in end-of-life care
Completion of Fundamentals of Hospice Palliative Care
Palliative Care certification an asset
Multifaceted and time management skills accompanied by flexibility
Excellent communication skills and ability to manage conflict
Able to function in an interdisciplinary team environment
Demonstrated critical thinking, assessment and problem resolution skills
Demonstrated interest in ongoing professional development
Proficient in use of Microsoft 365
