Position Summary:
As part of an interdisciplinary team of health care professionals in primary care, the Clinical Pharmacist supports patient care and the delivery of programs through chronic disease management, optimizing medications, and providing patients with resources and education for health promotion. This role includes specialized support for patients with memory‑related concerns and respiratory programs, contributing to the diversification of services for high‑risk patient populations.
Key Responsibilities:
- Provide individual patient medication assessments to identify, prevent, and resolve drug‑related problems.
- Support specialized medication assessments for patients receiving memory and geriatric care, including polypharmacy and deprescribing considerations.
- Support respiratory programs with medication optimization and patient education.
- Provide evidence‑based, unbiased drug information to the interdisciplinary team.
- Communicate prescribing recommendations to improve efficacy, safety, and patient adherence.
- In consultation with prescribers, develop patient‑centred care plans to reduce use of high‑risk medications.
- Collaborate with clinical working groups to develop patient‑centred care plans.
- Contribute to chronic disease management initiatives and quality improvement projects.
Qualifications:
- Current license with the Ontario College of Pharmacists (Part A).
- Baccalaureate of Science in Pharmacy; PharmD preferred.
- Strong understanding of evidence‑based and best practice pharmacy guidelines.
- Strong interpersonal, problem‑solving, organizational, and communication skills.
- Experience providing patient‑centred care and unbiased drug information.
- Strong project management or program development skills.
- Experience working within an inter‑professional health care team.
- Proficient in Microsoft Office and drug database programs.
