As a Wellness Supervisor (RPN) at V!VA Leaside, you will play a key leadership role in overseeing the coordination, quality, and delivery of care services provided by the Wellness Team. Through your clinical expertise, leadership, and commitment to exceptional care, you will help ensure Community Members receive exceptional care while promoting a culture of accountability, teamwork, and continuous improvement.
Reporting to the Wellness Manager, you will provide leadership and support to Wellness Leads, Unregulated Care Providers (UCPs), and Personal Support Workers (PSWs). You will collaborate with Community Members, families, healthcare professionals, and Team Members to coordinate care, support wellness initiatives, ensure regulatory compliance, and contribute to an exceptional Community Member experience.
All care services are delivered in accordance with individualized Care Plans, V!VA policies, and applicable legislative and regulatory requirements.
- Provide day-to-day leadership, guidance, and support to the Wellness Team, including Wellness Leads, UCPs, and PSWs.
- Foster a positive, accountable, and collaborative team environment focused on exceptional Community Member care.
- Monitor Team Member performance and provide coaching, mentorship, recognition, and constructive feedback.
- Mentor, coach, and support Team Member development through ongoing feedback, education, and performance discussions.
- Coordinate department scheduling to support operational needs and appropriate staffing levels.
- Participate in recruitment, interviews, onboarding, orientation, and training of Wellness Team Members.
- Provide leadership and operational oversight in the absence of the Wellness Manager.
- Support shift coverage as required to ensure continuity of care and service delivery.
- Collaborate with the Wellness Manager to oversee Community Member care and services.
- Conduct Wellness Assessments and participate in the development, implementation, and evaluation of individualized Care Plans.
- Coordinate and lead care conferences with Community Members, families, and healthcare providers.
- Liaise with physicians, pharmacies, hospitals, and other healthcare professionals to support continuity of care.
- Receive, transcribe, and action physician and prescriber orders in accordance with organizational policies and professional standards.
- Support Community Members and families by providing health education, guidance, and assistance with healthcare-related needs.
- Assist with assessments for prospective and existing Community Members, including move-in assessments when required.
- Support the daily operations of the Wellness Department and collaborate closely with Wellness Leads, UCPs, and PSWs.
- Administer medications and ensure the safe handling, storage, and documentation of medications, including narcotics and controlled substances.
- Provide professional nursing care in accordance with scope of practice, Care Plans, and physician orders.
- Respond to Community Member emergencies and coordinate appropriate follow-up and emergency response when required.
- Support vaccine clinics, wellness initiatives, health promotion activities, and clinical programs within the Community.
- Ensure proper utilization, maintenance, and storage of wellness supplies and equipment.
- Maintain accurate health records and ensure documentation meets professional, organizational, and regulatory standards.
- Conduct monthly, quarterly, and annual audits as directed.
- Support compliance with the Retirement Homes Act, RHRA requirements, College of Nurses of Ontario standards, and V!VA policies.
- Participate in and lead quality improvement initiatives, committees, and wellness programs.
- Provide oversight and support for Infection Prevention and Control practices within the Community.
- Contribute to departmental goal setting and achievement of organizational objectives.
- Demonstrate V!VA's Orange Dot Values in every interaction.
- Foster positive relationships with Community Members, families, visitors, healthcare partners, and Team Members.
- Participate in Community activities, events, training sessions, committees, and daily Making Today Great meetings.
- Promote a culture of respect, collaboration, accountability, and exceptional customer service.
- Complete special projects and additional responsibilities as assigned.
- Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
- Minimum of 5 years' nursing experience in retirement living, long-term care, seniors' care, or a related healthcare environment.
- Demonstrated supervisory, leadership, or team management experience in a healthcare, retirement living, or seniors' care environment is preferred.
- Strong clinical assessment, critical thinking, and care planning skills.
- Knowledge of Microsoft Office, including Outlook, Word, Excel, and Teams.
- Strong communication, customer service, and interpersonal skills.
- Ability to work independently while contributing to a collaborative team environment.
- Ability to maintain confidentiality and exercise sound professional judgement.
- Able to effectively communicate in verbal and written English.
- Valid CPR/First Aid Certification required.
- Smart Serve Certification is considered an asset.
- Valid Vulnerable Sector Screening (VSS) required.
- Ability to safely lift, carry, push, and move up to 50 lbs.
- Ability to work flexible hours, weekends, statutory holidays, and outside regular shift hours as required.
- Available for call-ins and emergency coverage when required.
- Valid driver's licence, automobile insurance, and access to a reliable vehicle.
