TLC Alert Nursing & Home Care is seeking Registered Nurses (RNs) and Registered Practical Nurses (RPNs) for an existing vacancy to conduct home care assessments for clients in Huntsville, Bracebridge, Orillia, and Gravenhurst. This is a fantastic opportunity for Nurses on their days off from their current job or for retired nurses who would like to continue to care for our clients in the community.
Key Responsibilities:
- Conduct comprehensive initial and follow-up home care assessments in clients homes.
- Evaluate clients physical, mental, and social needs to determine appropriate care.
- Develop and update individualized care plans based on assessment findings.
- Communicate assessment results with clients, families, and the care team.
- Identify changes in client condition and provide timely recommendations.
- Maintain accurate and timely documentation in line with CNO standards
Qualifications:
- Current RPN or RN license in good standing.
- Experience in home care, community health, or related nursing fields.
- Experience using AlayaCare is a strong asset but not required.
- Strong assessment, critical thinking, and communication skills.
- Comfortable working independently in clients’ homes.

Requirements:
· RPNs must be in good standing with CNO
· Current CPR and First Aid Certificate
· Clear Police Vulnerable Sector Check within the last 6 months
· TB test within the last 6 months
· Proof of 2nd dose of Covid-19 vaccination
*Proof of Booster shot
· Must be familiar with online charting (eg. MEDITECH, e-MAR, Point-Click-Care)
. N95 Mask Fit
· Home or community care experience an asset
· Should be able to work in a fast-paced, busy environment
· Must be energetic and compassionate
