Job Overview
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team. This position requires an RPN who is available to work evening and night shifts, as well as some weekends, to support the needs of our long-term care partners and the residents they serve. The ideal candidate will provide high-quality, resident-centred nursing care in long-term care homes. This role requires a strong foundation in nursing practices, excellent communication skills, sound clinical judgement, and the ability to work effectively within interdisciplinary teams. The RPN will be responsible for delivering personalized care to diverse patient populations, including seniors, children, and individuals requiring hospice or dementia care.
Responsibilities
- Administer medications and treatments accurately in accordance with physician orders and established protocols.
- Monitor patient vital signs and assess health status regularly to identify any changes or concerns.
- Assist with patient mobility, hygiene, and daily living activities to promote comfort and well-being.
- Document patient information meticulously using electronic health records such as Epic or Cerner systems.
- Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
- Provide specialized care for patients with dementia, hospice needs, or acute medical conditions.
- Perform phlebotomy procedures and laboratory tests as required.
- Educate patients and their families about health conditions, medication management, and post-discharge care instructions.
- Ensure compliance with infection control standards and safety protocols at all times.
- Participate in case management activities to coordinate comprehensive patient care across different services.
Skills
- Proven experience in nursing home, hospital, or acute care settings; experience in dementia care, pediatrics, or hospice is a plus.
- Strong knowledge of medical terminology, medication administration, and patient care procedures.
- Familiarity with electronic health records such as Epic or Cerner systems.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to work independently as well as part of a multidisciplinary team in fast-paced environments.
- Compassionate approach with a focus on delivering personalized patient-centered care.
- Ability to handle laboratory procedures like phlebotomy confidently when needed.
- Experience working with diverse populations including seniors, children, and individuals requiring specialized care such as pain management or case management services.
