Yorktown Family Services is a community-based non-profit social services agency dedicated to providing effective, accessible, quality mental health treatment, prevention, and outreach services to children, youth women, and families in the former City of York. We are uniquely organized to deliver services through two independent but integrated organizations: Infant, Child, and Youth Mental Health & West Toronto Youth Hub — a children’s mental health center, and Violence Against Women Services — a shelter for women and their children who are fleeing an abusive relationship.
Primary Function:
Yorktown Family Services is seeking a clinical therapist with Care Navigation experience to join our programs of dedicated professionals at Yorktown.
Yorktown requires a skilled family and child therapist fluent in English to provide clinical services to a diverse multicultural population offering services to individual, family, group, and Rapid Access Therapy services to a complex and multi-cultural clientele. This candidate will assist with triage support to youth, young adults, and their families (ages 17 to 29 years) who require complex care mental health and social service support within the framework of an integrated care team at Yorktown and linking them to outside external services.
Responsibilities:
A primary component of this position will be providing trauma-focused (TF) Cognitive Behavior Therapy (CBT) and mental health interventions to youth and their families. The successful candidate will be responsible for treatment services including individual, family, and/or group counseling, brief therapy with children, youth, and families, Trauma Assessments, and TF-CBT Treatment.
- Act as the client advocate; taking and receiving referrals; prioritizing needs and ensuring a smooth delivery of appropriate service.
- Assist with completing registrations for walk-in clients
- Provide coverage for walk-in shifts when required
- To act as a single point of contact for integrated care services.
- Work closely with healthcare partners to support the success of patients exiting hospital psychiatric settings and living in the community.
- Services may be delivered virtually and onsite at one of our three locations or in the community as needed.
- To support general intake functions when the intake worker is on vacation
- Provide brief SFBT counseling within our walk-in and virtual Rapid Access Therapy and at the West Toronto Youth Hub clinic, ‘What’s up’ walk-in clinic, and in the community as needed.
- Work within an integrated care health team as the mental health service provider.
- To understand the principle of assessment for vulnerable clients with complex mental health and social service needs and support referrals through assessments to the most appropriate team.
- Engage in service coordination and case consultation meetings with partnering agencies and external services to ensure successful service transitions
- Administer program and process evaluations and data collection for internal and external reporting.
- Present clients at case review meetings.
- Provide triage support for our walk-in clients by supporting them in completing registration forms and providing clients with an initial assessment with a focus on social service support and preparing them for a therapy session.
- Provide Care Navigation services for unattached clients who use our Urban Tele-Mental Health Program.
- Ensure all relevant documentation/records are maintained and kept in a secure location
- Initiate and maintain regular contact with youth and families
- Ensure agency health and safety protocols are followed.
- To provide emotional support for vulnerable clients in crisis and timely onward referral.
- Bridge clients from various sectors to mental health and social service supports.
- Work with Hospitals, Community Health Centers, and Physicians to support transitions from primary health to community-based child and youth care organizations.
Qualifications:
- M.S.W., or equivalent Master with several years of clinical experience grounded in a strengths-based, systemic perspective with youth aged 12 – 29 and their families
- Registration or eligibility for registration with a relevant college is required
- Minimum two years of relevant children and youth mental health experience with a preference for knowledge of and experience using evidence-based clinical models such as CBT and TF-CBT, goal-focused and brief therapy models of service provision
- Strong knowledge of and experience working with youth who are experiencing challenges relating to mental health, addictions, trauma, relationship violence, human/sex trafficking, and/or may be engaged in criminal activity, youth who have multiple risk factors, limited protective factors and have been engaged in specific incidents (e.g. fight at school, sibling in conflict with the law) that further increase their risks
- Knowledge of and sensitivity to the life experiences and needs of youth in the identified communities combined with community development experience is an asset
- Knowledge of and demonstrated skill in youth engagement principles and resiliency models
- Strong knowledge and experience working in the West end of Toronto including within NIAs (Neighbourhood Improvement Areas), as well as other identified communities and/or populations
- Demonstrated ability to work cooperatively and negotiate effectively within a multi-disciplinary team of service providers as well as the ability to work independently
- Enthusiasm and skill working to support change with a multi-challenged, diverse client population
- Excellent oral and written communication skills
- Superb interpersonal and organizational skills
- Demonstrated skills and experience working in collaboration and partnership with other service providers in the community
- Ability to work flexible hours including evenings is essential
- Access to a reliable vehicle is a definite asset
- Fluency in a second language is an asset
- Vulnerable Sector Screening is required
