Position Summary
This position is designed for a highly motivated Personal Support Worker (or similarly experienced health care professional) who is looking to grow into a regional clinical support and quality improvement role. Working directly with the Regional Health Care Director, the Clinical Care Coordinator will support multiple retirement residences across Eastern Ontario by improving resident care, documentation, inspection readiness, education, and standardized clinical processes.
This is not a traditional bedside role. It is an opportunity to develop into a regional leadership position while working closely with senior leadership. A company vehicle is provided.
Key Responsibilities
· Travel between retirement residences to provide hands-on support to leadership teams.
· Assist with resident care reviews, documentation reviews, care plan follow-up, and resident care binders.
· Identify trends related to resident safety, falls, documentation, medication practices, and quality of care.
· Support RHRA inspection readiness and follow-up on action plans.
· Provide education and coaching to PSWs and residence staff.
· Work with the Regional Health Care Director to develop standardized tools, checklists, policies, and quality improvement initiatives.
· Prepare summaries of site visits, observations, and recommendations.
· Participate in Teams meetings and collaborate across multiple homes.
Who We're Looking For
· Personal Support Worker (PSW) or healthcare professional with strong retirement home, long-term care, assisted living, or community care experience.
· Leadership experience (formal or informal) is an asset.
· Excellent communication, organization, and time management skills.
· Comfortable using technology including Microsoft Office, Teams, and electronic documentation systems.
· Valid Ontario driver's licence and willingness to travel throughout Eastern Ontario (company vehicle provided).
· Eager to learn, accept coaching, and grow into a regional leadership role.

Compensation
Salary: Starting at $55,000 annually (commensurate with experience).
Company vehicle provided. Full-time. In-person.
Pay: $55,000.00 per year
