Position Location: Thunder Bay, ON
Position Overview:
Under the direction of the Substance Use Health Manager, the Wellness Continuum Coordinator acts as the primary point of contact for service inquiries and referrals to the Day/Evening Adult Treatment Program. This position is an integral member of the interdisciplinary treatment team and is responsible for coordinating intake, assessment, pre-treatment readiness, and continuing care supports for adult clients. The Wellness Continuum Coordinator supports evidence-informed, trauma-informed, culturally responsive, and recovery-oriented treatment services that address substance use and co-occurring mental health needs.
Qualifications and Requirements:

Minimum of a diploma or degree in Social Work, Substance Use Health, Counselling, Psychology, or a related field. Registration or eligibility for registration with a relevant professional body is considered an asset.

Strong assessment, intake, and case coordination skills

Effective group facilitation and client education abilities

Excellent written and verbal communication skills

Sound knowledge of substance use treatment, recovery processes, and harm-reduction approaches

Ability to work independently and collaboratively within a multidisciplinary team

Strong organizational, problem-solving, and decision-making skills
-
Familiarity with relevant legislation, including the Mental Health Act and PHIPA
-
Cultural safety awareness and the ability to provide services in an inclusive and respectful manner

Computer proficiency and ability to work with electronic databases and documentation systems

satisfactory Criminal Records Search.

must possess a valid Standard First Aid/CPR Certificate or be willing to obtain one

ability to work flexible hours as determined by the Family Healing Manager or designate in relation to service requirements

must possess a Class F Driver’s License or be willing to obtain a Class F license and have access to a personal vehicle
-
must complete a medical and receive the required immunizations as recommended by the local medical officer of health.
Hours of Work: 32.5 hours per week
