Elevate NWO is seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our multidisciplinary Community Health Services team.
This position provides nursing care, testing, treatment support, health education, and clinical follow-up through our clinic, Drop-In Centre, outreach, and community programs. Working alongside our Nurse Practitioner and interdisciplinary team, you'll help improve access to low-barrier, client-centred health care for individuals experiencing complex health and social challenges.

What You'll Do
- Provide nursing care, treatment support, health education, and clinical follow-up.
- Conduct point-of-care and laboratory-based testing for HIV, HCV, STBBIs, and related health concerns.
- Perform venipuncture, specimen collection, and other clinical procedures within the RPN scope of practice.
- Support client assessment, care planning, treatment monitoring, and referrals in collaboration with the multidisciplinary team.
- Provide pre- and post-test counselling and harm reduction education.
- Participate in community outreach, testing events, and health promotion initiatives.
- Maintain accurate clinical documentation using electronic medical record systems.
- Collaborate with internal staff, community partners, and other health-care providers to support continuity of care.
Working Environment
This is a community nursing position with a diverse and dynamic practice environment. Work is performed in our clinic, Drop-In Centre, outreach settings, partner agencies, clients' homes, temporary living settings, and other community locations.
No two days are the same. The successful candidate must be comfortable working with individuals experiencing homelessness, substance use, mental health concerns, poverty, trauma, and other complex social and health needs. The role includes participation in overdose response, crisis situations, and other unpredictable clinical environments while working within established organizational policies, clinical protocols, and CNO standards.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario as a Registered Practical Nurse.
- Two to three years of nursing experience preferred, ideally in community health, primary care, harm reduction, outreach, infectious disease, or a related setting.
- Experience with phlebotomy, point-of-care testing, and specimen collection.
- Knowledge of HIV, Hepatitis C, STBBIs, harm reduction, substance use, and health equity.
- Excellent communication, clinical judgment, and organizational skills.
- Commitment to trauma-informed, culturally safe, and anti-stigma practice.
- Valid Ontario Class G driver's licence; access to an insured vehicle is an asset.
- Successful completion of a Vulnerable Persons Criminal Reference Check.
