Position Overview
Physiotherapy is about far more than treating injuries or prescribing exercises.

As a Physiotherapist – Community-Based Care, you will provide evidence-informed, client-centred physiotherapy services within clients’ homes and communities.
You will collaborate with clients, families, caregivers, referral partners, Physiotherapist Assistants, and interdisciplinary teams to improve mobility, reduce risk, restore function, and support continued progress.
Areas of Practice
Your referrals may include:
- Mobility, gait, balance, and transfer assessments
- Hospital-to-home transitions and post-acute rehabilitation
- Falls prevention, frailty, and aging in place
- Therapeutic exercise, strengthening, conditioning, and functional retraining
- Musculoskeletal, orthopaedic, and post-surgical rehabilitation
- Neurological rehabilitation and complex mobility
- Pain management and self-management education
- Cardiorespiratory rehabilitation, chronic disease, and deconditioning
- Mobility aids, transfer equipment, and Assistive Devices Program assessments, where authorized
- Workplace rehabilitation and return-to-function services
- Client, family, and caregiver education
No Physiotherapist is expected to practise across every service area described in this posting.
Referrals are thoughtfully assigned based on your demonstrated competence, experience, professional interests, applicable authorizations, geographic availability, and development goals.

Who We Are Looking For
We are looking for Physiotherapists who combine compassion with sound clinical judgment and who genuinely enjoy helping people regain movement, rebuild confidence, and rediscover the activities that matter most to them.
Community experience is valued, but it is not the only path to success.
We welcome applications from:
- Experienced community Physiotherapists
- Hospital, acute care, or inpatient rehabilitation Physiotherapists interested in transitioning into community practice
- Private practice, outpatient, primary care, long-term care, and rehabilitation clinicians
- Physiotherapists with experience in geriatrics, neurology, orthopaedics, cardiorespiratory care, chronic disease, pain, falls prevention, mobility, insurance, or complex care
- Early-career Physiotherapists and new graduates who are ready to learn, grow, and build rewarding careers in community practice
We are looking for curious, compassionate, accountable, and adaptable clinicians committed to purposeful rehabilitation that is practical, measurable, and meaningful.
Qualifications
Successful candidates must have:
- A degree in Physiotherapy or Physical Therapy from an accredited or recognized program
- Current registration in good standing with the College of Physiotherapists of Ontario
- Eligibility to practise physiotherapy in Ontario
- Professional liability insurance
- Strong communication, documentation, organization, clinical reasoning, and exercise-prescription skills
- A valid driver’s licence and access to reliable transportation for community visits, where required
Experience Considered an Asset
Experience in any of the following areas will be considered an asset:
- Home and community care
- Ontario Health atHome
- Hospital discharge planning and post-acute transitions
- Gait, balance, falls prevention, and mobility rehabilitation
- Therapeutic exercise, conditioning, and functional retraining
- Neurological, orthopaedic, geriatric, or cardiorespiratory rehabilitation
- Assistive Devices Program assessments and mobility equipment
- Pain management, chronic disease, or deconditioning
- Physiotherapist Assistant supervision
- Automobile insurance, WSIB, workplace rehabilitation, or return-to-function services
Experience in every area is not required.
