Position Overview:
We are currently looking for a full-time Behavioural Health Consultant - Registered Nurse (BHC - RN). The Behavioural Health Consultant (BHC) role is grounded in the Primary Care Behavioural Health model and provides brief, evidence-based behavioural and mental health support to a wide range of patients within primary care. Working as part of the multidisciplinary team, BHCs deliver functional assessments and focused interventions for concerns related to mental health, substance use, and life stressors or crises. The BHC will be an integrated team member working to their full scope of practice within the primary care setting collaborate closely with the wider team to support behaviour change and enhance team capability. This role will report to a primary care manager.
The BHC will work with the family primary care providers family in the community, other health professionals and local and regional programming in a multidisciplinary environment. The goals is to ensure a high quality of evidence-based health care is delivered to the practice population.
Key Roles and Responsibilities
- To work collaboratively in a patient centred, integrated team based primary care practice setting.
- To participate in the care of individual patients, practice populations and associated communities.
- To take on a solution-focused, goal-directed approach with clinic patients
- To assist in development of brief intervention strategies with clinic patients
- To make professional, competent and autonomous decisions with patients as a member of the multidisciplinary team.
- To facilitate groups offered at our central office.
- To act as a resource to other multidisciplinary team members, healthcare institutions and community agencies or programs.
- To ensure accurate, objective and timely clinical documentation in the appropriate medical record.
Experience, Skills & Qualifications:
- Bachelor’s Degree in Nursing or Registered Psychiatric Nurse
- Registration with relevant College as recognized by the Health Professions Act
- Current Basic Life Support (BLS) for Healthcare provider
- Understanding of the Primary Care Behavioural Health (PCBH) Model
- Direct experience supporting individuals with mental health challenges
- Reliable Transportation
- Punjabi language skills an assest
- Understand the Alberta privacy laws and act according to those laws.
- Should behave responsibly, be accountable and have excellent written and verbal skills, problem solving/critical thinking skills.
- To support the vision and mission of the Primary Care Network. Successful candidate will be required to provide a criminal record check
Previous experience:
- Primary care setting
- Electronic Medical Record
- Group facilitation/instruction
- Health behavior change methods (eg Motivational Interviewing)
