Schedule: overnight shifts. Shifts would be 2200-0600 or 2300-0700.
Full time position.
What You’ll Be Doing:
- Deliver comprehensive nursing care to patients in home settings, focusing on individualized treatment plans.
- Conduct thorough assessments of patients' health status and develop appropriate nursing interventions.
- Collaborate with multidisciplinary teams to coordinate patient care and ensure continuity of services.
- Educate patients and their families on health management, medication administration, and home care practices.
- Monitor patient progress and adjust care plans as necessary based on clinical evaluations.
- Maintain accurate documentation of patient care activities using electronic health record systems, such as Epic.
- Provide support for laboratory tests and procedures as needed, ensuring adherence to safety protocols.
What we look for in our candidates:
- Previous experience with pediatrics is highly desirable.
- Strong background in patient care with a focus on community home health
- Excellent communication skills and the ability to work collaboratively with healthcare teams and families.
- Experience working with children and understanding their unique healthcare needs is beneficial.

Pay: $34.00-$36.00 per hour
