Reporting to the Director of Community Health, the Chronic Disease Consultant provides leadership, guidance and support regarding the development and implementation of an integrated chronic disease framework and associated strategies, action plans and initiatives. The Chronic Disease Consultant will lead planning, program development, innovation, policy, evaluation and monitoring processes related to the prevention (primary and secondary) and management of chronic disease within the department. The incumbent will also provide high quality advice and assistance to support operations to be responsive to client needs and focusing on improved health outcomes. Duties include
Duties will include but are not limited to:
- Conducting reviews of current health system plans and strategies with a view to implementing the Chronic Disease Framework and associated strategies, plan, etc.;
- Managing consultative processes to ensure appropriate and diverse key stakeholder involvement in the planning process and in the evaluative component;
- Conducting comprehensive literature reviews and analysis on issues, trends, policies and system directions; and consults with content experts and opinion leaders, other jurisdictions, other government departments;
- Provides advice, assistance and direct support to the Director of Community Health, Executive Director, Assistant Deputy Minister, Deputy Chief Health Officer and their management teams in the implementation of the Chronic Disease Framework;
- Developing and providing policy guidance on chronic disease management programs and services to ensure it aligns with the provincial framework;
- Coordinating the development and maintenance of an efficient data collection and information management system to support evaluation to assist informing policy and program development;
- Collaborating with Health PEI and other partners on the further development and implementation of integrated chronic disease prevention and management programs;
- Supporting Health PEI and other partners in the planning, development, integration, implementation and evaluation of chronic disease prevention and management approaches and strategies;
- Participate in the development disease specific Chronic Disease Prevention and Management initiatives;
- Initiating and leading processes which facilitate effective and positive communications with other governmental and non-governmental organizations to ensure an integrated approach to legislative and regulatory compliance;
- Representing the Department of Health and Wellness at various provincial and territorial meetings, advisory committees, and working groups;
- Developing and/or delivering written and visual communication products for social media, websites and presentations;
- Responding quickly and competently to high priority questions and assignments.
- Other duties as required.
Minimum Qualifications:
- Baccalaureate Degree in a health or social discipline supplemented with extensive experience working in the area of integrated chronic disease prevention and management programs and initiatives. Master’s degree is considered as asset.
- Considerable leadership/management experience including leadership, budget management, risk management, strategic and operation planning, policy and program development, and evaluation and processes.
- Knowledge of policy analysis and development.
- Demonstrated equivalencies will be considered.
- Strong project management skills required.
- Proven organizational and communication skills including conflict resolution.
- Experience in developing and maintaining successful partnerships across sectors and proven ability to work effectively in a collaborative team environment with a variety of disciplines.
- Working knowledge of health sector, is required.
- Excellent oral and written communication skills.
- Must possess a valid driver’s license and be willing to travel on a regular basis to other areas of the province for meetings as required.
- The successful candidate must provide a satisfactory Criminal Record Check prior to commencement of employment
- Familiar with basic computer and software programs (Corel, MS Suite, Excel, wording processing, spreadsheets, presentations, etc).
- Basic office equipment such as printers, copiers, faxes, business and cellular phones, etc.
- Familiar with audio-visual equipment.
- Additional relevant education and experience will be considered an asset.
