Summary of Job:
Acting as the Most Responsible Provider (MRP) for a specific group of patients, the selected Nurse Practitioner will provide comprehensive primary care to patients across all ages of life within a rural primary care clinic. Working closely with the other allied health providers and physicians this position is well suited to an experienced NP who is looking for an opportunity to work to full scope within a supportive team environment.
Responsibilities:
● Deliver comprehensive care (including but not limited to medication reconciliation, prescriptions, ordering lab work, administrative tasks, physical examinations) to patients.
● Liaise, as required, with patient physicians and other community partners to help address patient needs.
● Educate patients on disease prevention and/or prevention of further disability.
● Maintain accurate and timely clinical documentation in our electronic health record system.
Qualifications:
● Current registration and in good standing with the College of Nurses of Ontario.
● Strong computer skills.
● Ability to work well independently and within an interdisciplinary team.
● Good organizational skills, accuracy and attention to detail.
● Strong interpersonal and communication skills, both written and verbal.
● Experience working in a community setting and with individuals with complex needs.
● Demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, and/or gender identity.
● Satisfactory Vulnerable Sector Check required.
● Valid driver’s license with $2 million vehicle liability insurance.
