Responsibilities
- Conduct thorough patient assessments, including history-taking and physical examinations for individuals across various age groups.
- Develop and implement personalized care plans that incorporate evidence-based practices and current medical guidelines.
- Manage medication administration, including injections, IV therapies, and pain management protocols, ensuring safety and compliance with regulations.
- Collaborate closely with multidisciplinary teams to coordinate case management efforts, especially in complex or chronic cases such as dementia or acute illnesses.
- Perform procedures such as phlebotomy, laboratory specimen collection, and wound care to support diagnostic processes and treatment efficacy.
- Utilize electronic health record system like Accuro for documentation, order entry, and communication with other healthcare providers.
- Educate patients and their families on health conditions, medication use, lifestyle modifications, and preventive care strategies to empower informed decision-making.
Qualifications
- Valid licensure as an Advanced Practice Registered Nurse (APRN) with certification as a Nurse Practitioner (NP).
- Proven experience working with children (pediatrics) and senior populations in clinical or long-term care settings.
- Extensive nursing background including hospital experience, nursing home care, assisted living environments, or home health services.
- Strong knowledge of medical terminology, medication protocols, and clinical procedures relevant to acute and chronic care management.
- Experience with electronic health record platform such as Accuro is highly desirable.
- Competency in pain management techniques, dementia care strategies, laboratory procedures, and case management practices.
- Excellent patient care skills with a compassionate approach that prioritizes safety, comfort, and dignity for all patients.
