Our Community Health Centre provides accessible, trauma-informed, culturally responsive primary care for individuals with complex medical and socio-economic needs, including those affected by poverty, homelessness, addictions, and chronic illness.
Job Summary
Jasper Place Wellness Centre Community Health Centre is seeking a Nurse Practitioner to join our interdisciplinary primary care team in Edmonton, Alberta. This role focuses on complex primary care, chronic disease management, and serving individuals facing barriers to accessing traditional health services.
This is an excellent opportunity for experienced Nurse Practitioners or New Graduates interested in community-based, equity-focused health care.
Key Responsibilities
- Perform comprehensive health assessments, diagnoses, and treatment plans
- Provide primary care services across the lifespan.
- Manage chronic diseases using evidence-based clinical guidelines
- Prescribe medications and order/interpret diagnostic tests within NP scope
- Develop, implement, and evaluate patient-centered care plans
- Collaborate with physicians, pharmacists, social workers, and outreach teams
- Support patients impacted by social determinants of health
- Maintain accurate clinical documentation and reporting
- Participate in interdisciplinary case conferences and care planning
Required Qualifications
- Active registration with the College and Association of Registered Nurses of Alberta (CARNA)
- Registered on the CARNA Nurse Practitioner – All Ages roster
- Immunization certification (required)
- Eligible to practice independently in Alberta
- Ability to provide proof of licensure and good standing

Job Type: Full-time
Pay: $75.00-$100.00 per hour
Expected hours: 1 per week
