PURPOSE OF THE ROLE: The CSN Liaison is responsible for the day-to-day care management as well as interviewing and assessing external candidates as potential Home Share Providers. The CSN Liaison ensures that Home Share Providers meet acceptable standards of service delivery.
QUALIFICATIONS:
- A minimum of a 2-year Community College Diploma directly related to the program area is preferred. A combination of education, experience, and aptitude will be considered;
- At least one year of experience working with adults with a developmental disability;
- Valid First Aid and CPR certification, and successfully clear a Criminal Record Check;
- Must hold a Class 5 Drivers License and have access to own reliable vehicle for daily use;
- Good command and understanding of verbal and written English communication skills;
- Demonstrated experience in Person and Family Centered Planning;
- Ability to build strong, collaborative relationships with caregivers and families;
- Ability to work independently and as part of a team;
- Excellent flexibility, problem solving, decision making and creative thinking skills;
- Experience providing behavioral supports;
- Proficiency with computers, including Microsoft Office Suite;
- Experience in operationalizing values and principles into high quality supports for individuals in the community;
- Experience in the non-profit sector is an asset; and
- Strong interpersonal communication, time management and organizational skills.
WAGE RATE: As per collective agreement at Grid Level 10 ($25.95-$29.76) plus 6% increase pending Collective Agreement ratification.
