Summary of Job:
The Nurse Practitioner - Primary Care will work collaboratively with the allied health professionals to provide comprehensive primary care services including health promotion, disease, and injury prevention, treatment, rehabilitation, and support for patients. This position involves seeing patients of all ages, across the lifespan and is ideally suited to an experience NP who can work to their full scope capable of managing a high volume of appointments with the expectation of seeing 16 – 20 visits per day.
Responsibilities:
- Deliver comprehensive care (including but not limited to medication reconciliation, prescriptions, ordering lab work, administrative tasks, physical examinations) to patients.
- Liaise, as required, with patient physicians and other community partners to help address patient needs.
- Educate patients on disease prevention and/or prevention of further disability.
- Maintain accurate and timely clinical documentation in our electronic health record system.
Qualifications:
- Current registration and in good standing with the College of Nurses of Ontario.
- Strong computer skills.
- Ability to work well independently and within an interdisciplinary team.
- Good organizational skills, accuracy and attention to detail.
- Strong interpersonal and communication skills, both written and verbal.
- Experience working in a community setting and with individuals with complex needs.
- Demonstrate sensitivity to diversity, whether related to race, religion, culture, sexual orientation, and/or gender identity.
- Satisfactory Vulnerable Sector Check required.
- Valid driver’s license with $2 million vehicle liability insurance.
