The Community Health Representative serves as a vital liaison between members of the Low German Speaking Mennonite community and health and human service agencies within Rural Public Health. This role supports individuals and families in navigating health and social services by providing culturally appropriate outreach, advocacy, education, and resource coordination. Through relationship building, health promotion, and community engagement, the Community Health Representative empowers clients to make informed decisions, practice self-care, access available services, and take an active role in improving their health and well-being. The successful candidate must be fluent in Low German (Plautdietsch) and possess a strong understanding of Low German Mennonite culture, values, and perspectives.
As a Community Health Representative, you will work in partnership with community health staff in the provision of screening programs, health education and communicable disease control, and enhance the community capacity through strong community partnerships.

Required Qualifications:
Completion of Grade 12.
Additional Required Qualifications:
The candidate will demonstrate experience working with members of the Low German Mennonite community and collaborating with community groups, organizations, and agencies. Strong oral and written communication skills are required, as well as knowledge of the health care system and health promotion and healthy living principles. The successful candidate will possess proven organizational, planning, and time management skills, with the ability to work both independently and collaboratively within a multidisciplinary team. Proficiency in Microsoft Word and Outlook is required, along with the ability to build and maintain positive working relationships with community members, stakeholders, and service providers.
Preferred Qualifications:
Knowledge of Low German Mennonite culture and the ability to communicate fluently in the Low German language (Plautdietsch). Previous work in health care, for example Health Care Aid.
