Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality nursing care to patients in various settings, including medical offices, hospitals, nursing homes, and senior care facilities. The ideal candidate will have experience in acute care and a strong ability to work with children and elderly patients, particularly those with dementia. This role requires effective communication skills, a commitment to patient care, and the ability to work collaboratively within a healthcare team.
Duties
- Administer medications and monitor patient responses to treatments.
- Assist in the development and implementation of patient care plans.
- Provide direct patient care, including bathing, dressing, and feeding as needed.
- Conduct routine assessments of patients' health status and report changes to the supervising nurse or physician.
- Maintain accurate patient records using electronic health record systems such as Cerner.
- Support case management by coordinating care among healthcare providers.
- Educate patients and their families about health conditions and treatment plans.
- Ensure compliance with safety protocols and infection control procedures.
Qualifications
- Valid LPN license in the state of practice.
- Experience in medical office settings or acute care preferred.
- Familiarity with dementia care practices is a plus.
- Previous experience working with children is desirable.
- Knowledge of case management principles is beneficial.
- Ability to work effectively in hospital or nursing home environments.
- Strong communication skills and the ability to build rapport with patients and families.
- Proficient in medication administration techniques.
