Overview
We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team to supervise and provide care in our new Memory Care Wing. The ideal candidate will have a strong background in patient care and possess the skills necessary to provide high-quality nursing services in . This role requires a commitment to enhancing the well-being of patients through effective nursing practices and collaboration with healthcare professionals.
Responsibilities
- Provide direct patient care, including administering medications and treatments as prescribed.
- Monitor and assess patient conditions, documenting changes in medical records.
- Assist in the development and implementation of individualized care plans for patients.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Educate patients and their families on health management, medication administration, and treatment plans.
- Support patients with activities of daily living
- Maintain accurate medical terminology in documentation and communication with healthcare staff.
- Participate in case management activities to coordinate patient services effectively.
Qualifications
- Experience in a retirement home, assisted living or nursing home is preferred;
- Familiarity with dementia care is an asset
- Valid CPR/First Aid Certification
- Proficiency in medication administration and knowledge of medical terminology is essential.
- Experience using electronic health record systems
- Flexibility to work a variety of shifts, including evenings, nights, weekends and holidays
- Strong interpersonal skills with the ability to work effectively with seniors
- A valid nursing license is required along with a commitment to ongoing professional development.
