Job Summary
We are seeking a dedicated and experienced Specialized Nurse Practitioner (SNP) to provide advanced patient care across diverse healthcare settings. The ideal candidate will possess a strong background in nursing, with expertise in working with children, seniors, and patients requiring acute or chronic care. This role involves collaborating with multidisciplinary teams to deliver comprehensive, patient-centered care, including pain management, case management, and specialized treatments. The SNP will play a vital role in improving health outcomes through clinical excellence and compassionate patient interactions.
Duties
- Conduct comprehensive assessments of patients across various age groups, including pediatrics and seniors.
- Develop and implement individualized care plans tailored to patient needs, including those in nursing homes, assisted living facilities, hospitals, and home care environments.
- Administer medications accurately following medical protocols and ensure proper documentation using electronic health records such as Epic and Cerner systems.
- Perform procedures such as phlebotomy, laboratory testing, and other diagnostic interventions.
- Manage cases involving dementia care, pain management, and chronic disease conditions.
- Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate effective treatment plans.
- Educate patients and families on health management strategies and medication regimens.
- Monitor patient progress and adjust treatment plans accordingly to optimize health outcomes.
- Maintain compliance with healthcare regulations and uphold high standards of patient safety and confidentiality.
Qualifications
- Valid Nurse Practitioner license with current certification in relevant specialties.
- Proven experience working with children, seniors, or in hospital settings; nursing home or assisted living experience is highly desirable.
- Strong knowledge of medical terminology, medication administration, and clinical procedures.
- Proficiency with electronic health record systems such as Epic and Cerner.
- Experience in acute care, senior care, pain management, case management, pediatrics, or home care is preferred.
- Excellent patient care skills with compassionate communication abilities.
- Ability to handle complex cases involving dementia care or laboratory procedures such as phlebotomy.
- Familiarity with medical office operations and laboratory environments is advantageous.

Pay: $55.00-$90.00 per hour
