Your Opportunity
We are hiring six (6) Registered Practical Nurses (RPNs) to join our growing team in a unique, high-impact role supporting family physicians. These are working 28 hours per week. This position is ideal for RPNs who enjoy working at full scope in a primarily administrative, non-procedural nursing role focused on clinical workflows and patient care management. You will provide high-volume clinical administrative support through EMR-based workflows, contributing directly to patient care by managing results, supporting clinical decisions, and improving care coordination. In this role, you will focus on inbox management, results follow-up, medication renewals, and chart optimization - helping physicians work more efficiently while ensuring patients receive timely, proactive care.

Key Responsibilities and Skills:
Inbox & Results Management
- Triage and manage physician EMR inboxes (OSCAR)
- Review and action laboratory and diagnostic results using established protocols
- Identify abnormal or urgent findings and escalate appropriately
- Communicate results to patients within scope and guidelines
- Prepare clear, actionable summaries to support physician decision-making
Medication Management
- Process medication renewal requests
- Conduct chart reviews to ensure safety and appropriateness
- Identify required monitoring (labs, follow-ups)
- Prepare renewals for physician approval or complete within scope
Clinical Documentation & Forms
- Complete complex forms (insurance, disability, and other medical documentation)
- Gather and synthesize relevant patient information
- Prepare drafts for physician review and sign-off
Chronic Disease & Preventive Care Support
- Identify care gaps (e.g., diabetes, hypertension, cancer screening)
- Support patient recall and follow-up workflows
- Monitor registries and patient cohorts
- Initiate protocol-based outreach to support preventive care
Chart Review & Optimization
- Perform chart clean-up and reconciliation
- Update problem lists, medications, and preventive care tracking
- Support pre-visit planning and continuity of care
- Contribute to workflow efficiency during physician absences or downtime
Other Duties as Assigned
Job Requirements:
- Diploma in Registered Practical Nursing, RPN Designation
- Current registration in good standing with the College of Nurses of Ontario (CNO)
- 3-5 years of progressive clinical experience (primary care experience considered an asset)
- Strong understanding of the Ontario healthcare system
- Experience working within an interdisciplinary healthcare team
- Knowledge of evidence-based primary care and chronic disease management
- Excellent written and verbal communication skills
- Strong organizational, time management, and critical thinking abilities
- High level of comfort working within EMR systems (OSCAR preferred)
- Ability to manage high-volume workloads with accuracy and efficiency
- Strong clinical judgment and decision-making within scope
- Detail-oriented with a high degree of accountability
- Adaptable, resourceful, and able to manage competing priorities
- Committed to patient-centered care and team collaboration
- Demonstrated professionalism, sound judgment, and ability to handle sensitive or complex situations
- Maintains strict confidentiality and professionalism at all times
