Your role:
As a Community Visiting Nurse (RPN), you’ll provide direct nursing care to clients in their own homes within the community. You’ll collaborate with interdisciplinary teams and families to develop care plans and deliver compassionate, professional care tailored to each patient’s needs.
We’re Looking For Someone Who Has:
✔ Current registration in good standing with the College of Nurses of Ontario (CNO)
✔ Diploma in Practical Nursing
✔ Personal vehicle and driver’s license
✔ Basic Life Support (CPR) certification
✔ Minimum of 2 years of experience in a community nurse role
✔ Strong communication, clinical assessment, and decision-making skills
✔ Compassionate, respectful, and adaptable approach to patient care
✔ Knowledge of electronic health records (AlayaCare, HPG)
✔ Willingness and ability to participate in the on‑call rotation as required
✔ Strong interprofessional collaboration skills to support comprehensive care planning
Additional Assets:
✔ Advanced wound care or medication administration experience
✔ Competency in head-to-toe assessments and nursing documentation
✔ Delegation of tasks experience
✔ Experience with Central Venous Access Devices (CVADs)
✔ Ongoing education in palliative care competencies (e.g., LEAP certification)
✔ Ability to conduct Lower Limb Assessments (LLA)
✔ Knowledge of pediatric home care and diabetes management
Previous experience supporting chemotherapy administration and management
✔ N95 Mask Fit Test Certificate, Vulnerable Sector Police Check, and COVID-19 vaccination (2 doses)
