Position Summary:
Physiotherapist: Mackenzie Health Community Stroke Rehab Clinic
We are currently seeking a Full time Physiotherapist to work in Community Stroke Rehab Clinic located in Vaughan (955 Major Mac). Physiotherapist will provide patient management through all stages of assessment, treatment, consultation and follow-up services to patients. The successful candidate will be familiar with working in an outpatient clinic setting.
As a successful candidate, you are a graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists of Ontario. You demonstrate excellent interpersonal, organizational and communication skills, both written and verbal, along with a high level of initiative and adaptability to rapidly changing demands.
Responsibilities:

As a Physiotherapist (PT) you will be conducting assessments & treatment plans
-
As a PT you will be formulating goals and recommendations
-
Planning and implementing patient centered, goal-directed treatment programs & working with Therapy Assistants
-
Assist in the promotion of physiotherapy services to external referral sources

Participating actively in the planning, implementation and evaluation of the program

Provide ad-hoc home based PT therapy as needed
Requirements:
-
Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team

Graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists Ontario

2 years of clinical experience in Stroke/Neurological patient populations is preferred

Experience in assessment and generating treatment plans for patients with stroke/neurological disorders
-
Knowledge of stroke best practices
-
Experience with Chronic disease management is a good asset

Sound knowledge of community resources is an asset
-
NDT training is an asset
-
Effective interpersonal, organizational and communication skills
-
Strong customer service focus and excellent problem-solving skills required

A positive work and attendance record is essential

Ability to adapt to new computer applications

CPR Level C + AED, and Standard/ Intermediate First Aide certificate

Valid G license required
-
Criminal background check, Vulnerable sector check and Immunization record (current)
