Job Title: Home Share and Community Inclusion Program Manager
Reports to: Executive Director
Job Summary:
In collaboration with the Executive Director, the Home Share/Community Inclusion Manager assumes the responsibility of the management and integrity of 28th Avenue Homes Ltd.’s Home Share and Community Inclusion Programs. Provides support and supervision by working directly with individuals, home share providers and community inclusion staff as required.
Advocacy at many levels and various forms of public awareness and education are a component of this position.
Duties and Responsibilities:
· Oversees, supervises, organizes, controls and directs the provision of services for the Home Share and Community Inclusion programs. Ensures effective program /service delivery consistent with agency mandate, policies and other requirements.
· Provides supervision, direction and support to Home Share Coordinator, Home Share Providers and Community Inclusion staff.
· Ensures all Home Share and Community Inclusion files are kept current and up to date with all required documentation.
· Provides advice and presents recommendations to the Executive Director on significant program and service issues.
· Determines resource and operational requirements for all Home Share and Community Inclusion areas of responsibility
· Advocates to ensure adequate support and funding to meet unique needs of each individual.
· Maintains confidentiality of privileged information which may not be shared. Demonstrates professional discretion when disseminating information.
· Engages proactively with Home Share Providers and Community Inclusion staff to resolve difficult or sensitive issues or problems.

· Experience in advocacy, plan development, conflict resolution, development of policies/ handbooks.
· Excellent presentation, public relations, oral, written and interpersonal communication
· Excellent administrative and organizational skills.
· Demonstrated teamwork, leadership and managerial skills.
· Well-developed planning, organizing, controlling and decision-making skills.
· Ability to deal tactfully with sensitive issues.
· Assures compliance with current CARF standards.
· Functions well under pressure while managing multiple concurrent projects and deadlines, including effectively managing stressful and emergency / crisis situations.
Education and Knowledge:
· Diploma in related human/social service field.
Training and Experience:
· 2+ years’ experience in home share/community inclusion
Equivalent combination of education, training, and experience may be considered.
Pay: From $37.00 per hour
Experience:
- management: 2 years (preferred)
Licence/Certification:
- Social Worker Licence (preferred)
