Job Summary
The Social Worker works in partnership with other members of the primary health care team to provide comprehensive, patient-centred care to the entire patient population. The Social Worker focuses on providing psychosocial assessment, counselling, brief intervention, care coordination, system navigation, and support to address the social determinants of health, with a goal of improving health outcomes, enhancing patient well-being, and facilitating access to community and health services.
Duties & Responsibilities
- Provide comprehensive psychosocial assessments of clients, taking into consideration mental health, social determinants of health, family and interpersonal relationships, housing, financial circumstances, cultural background, and other factors impacting health and well-being.
- Provide individual and family counselling, brief interventions, supportive counselling, and psychoeducation within the scope of social work practice.
- Support clients experiencing mental health concerns, life transitions, grief and loss, family or relationship challenges, social isolation, and other psychosocial issues.
- Develop patient-centred care and support plans in collaboration with clients, families/caregivers, and members of the primary health care team.
- Provide care coordination and system navigation, connecting clients with appropriate health, social, housing, financial, legal, community, and other support services.
- Advocate for clients and support equitable access to health and social services, with particular attention to barriers related to social determinants of health.
- Facilitate referrals and maintain collaborative relationships with community agencies, hospitals, mental health services, and other service providers to support continuity of care.
- Participate as an active member of the interdisciplinary primary care team, contributing social work expertise to patient care planning and case discussions.
- Provide consultation and support to primary care providers and other members of the health care team regarding psychosocial concerns and community resources.
- Facilitate and/or co-facilitate group programs, workshops, and patient education initiatives related to mental health, wellness, coping, social supports, and other identified patient needs.
- Maintain accurate, timely, and confidential clinical documentation in accordance with professional standards, organizational policies, and applicable legislation.
- Ensure continuity of care through timely communication with clients, primary care providers, caregivers, and other service providers, as appropriate and with appropriate consent.
- Participate in quality improvement initiatives, team meetings, and other activities that support the delivery of accessible, integrated, and patient-centred primary health care.
- Perform other related duties within the scope of social work practice as required.

- Salary: $77,673 per year
- Anticipated Start date November 2/2026.
- Professional Registration: Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
