CANES Community Care – Transitional Care Program
Locations: Brampton (38 beds) & Bolton (35 beds)
Commitment: Approximately 1 day per week

CANES Community Care is dedicated to providing high-quality, compassionate care to clients within their home and in the community. Our Transitional Care Program is designed for medically stable clients who require short-term support, while awaiting discharge to the community. Through a collaborative, client-centered approach, we focus on promoting recovery, ensuring continuity of care, and helping clients transition safely and successfully to their home or next level of care.
Position Overview
- We are seeking a part-time Physician to support our Transitional Care Program across our Brampton and Bolton sites. This role involves providing ongoing medical oversight to a short-stay population, collaborating closely with nursing teams to deliver patient-centered care.
- Remuneration is fee-for-service as set by the OHIP Schedule of Benefits and Fees.
Key Responsibilities
- Conduct weekly on-site rounds at one or both locations
- Review client updates prepared by nursing staff
- Provide virtual or telephone consultations as required
- Offer after-hours support to nursing staff for urgent/emergency concerns
- Support outbreak management when necessary
- Perform client assessments and ongoing health evaluations
- Initiate and manage referrals to specialists
- Order and review laboratory requisitions
- Conduct medication reviews
- Facilitate palliative care referrals as appropriate
Qualifications & Requirements
- Certificate of Registration from the College of Physicians and Surgeons of Ontario (CPSO) (required)
- Prior healthcare experience is an asset
- Strong communication skills in English (required)
- Additional languages are considered an asset, reflecting our diverse client population
- Demonstrated interest in geriatric and community-based care
