Key Responsibilities
- Assist clients with personal care, bathing, dressing, grooming, and toileting
- Provide mobility and transfer assistance
- Prepare meals and assist with feeding when required
- Perform light housekeeping and assist with laundry
- Assist clients to and from appointments/ outings
- Provide companionship and emotional support
- Document care provided and report any health care changes
- Follow individualized care plans and safety protocols
- Administer medications as prescribed
Requirements
- Care Aide, LPN or equivalent education, training, or experience preferred
- Experience supporting seniors or individuals with Dementia and/or disabilities
- Clear Criminal Record Check and Vulnerable Sector
- Reliable transportation and valid driver’s licence required
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Flexibility to work a variety of visits including evenings and weekends

Job Summary
We are seeking compassionate and dedicated Care Aides or Licensed Practical Nurses (LPNs) to join our healthcare team in <LOCATION>. This role involves providing high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. The ideal candidate will have experience in diverse care areas such as dementia, pediatrics, senior care, hospice, and acute care. You will play a vital role in supporting patient well-being through attentive and professional nursing services.
Duties
- Deliver comprehensive patient care, including assistance with activities of daily living and medical needs
- Administer medications and treatments accurately following physician orders
- Monitor patient health status and report changes to the healthcare team
- Assist with medical procedures such as phlebotomy and laboratory specimen collection
- Utilize electronic health record systems like Epic or Cerner for documentation and case management
- Support patients with dementia, hospice, pediatric, or senior care needs with empathy and professionalism
- Collaborate with multidisciplinary teams to develop and implement individualized care plans
- Maintain a safe, clean, and comfortable environment for all patients
- Educate patients and families about health conditions and treatment plans
Requirements
- Valid LPN license or relevant certification; Care Aide certification preferred
- Prior experience in nursing home, hospital, acute care, or home care settings is highly desirable
- Knowledge of medical terminology, medication administration, and patient safety protocols
- Experience with electronic health records such as Epic or Cerner is a plus
- Ability to work effectively with diverse patient populations including children, seniors, or hospice patients
- Strong communication skills and compassionate bedside manner
- Ability to work independently and as part of a healthcare team
- Flexibility to adapt to various care environments and patient needs
