Overview
Staff Relief Healthcare Services is hiring experienced community/home care nurses to conduct in-home client assessments across Ontario.
This independent role is ideal for senior nurses who value flexibility, autonomy, and clinical decision-making.
What You’ll Do
- Conduct in-home client assessments and reassessments
- Evaluate physical, cognitive, and functional status
- Determine required nursing procedures and level of care
- Assess need for PSW services or skilled nursing support
- Develop and document care plans
- Submit clear, accurate clinical reports
- Communicate recommendations to internal coordination team
What We’re Looking For
- Active RN or RPN license in good standing with the College of Nurses of Ontario
- Minimum 2+ years nursing experience (community/home care strongly preferred)
- Strong independent assessment and clinical judgment skills
- Confident decision-maker with excellent documentation ability
- Valid Ontario Driver’s License and reliable vehicle
- Current CPR certification
