Yellowhead Community Services (YCS) is looking for an individual, couple or family willing to become the Home Share Provider for a social, easygoing and good-natured woman in her sixties. This individual does have diverse abilities and health challenges predominantly related to aging and is wishing to continue to reside in the Barriere or Clearwater area.
As a Home Share Provider, you will provide ongoing support and guidance building life skills, assisting with meal preparation, accessing community resources, and services. You will encourage inclusion within the home such as sharing meals, sharing hobbies, playing cards etc. as well as supporting inclusion in community. This could look like attending local events, beach days at the lake, going out for coffee, volunteering and supporting connection with friends and peers as examples.
You will be responsible for the safety, support, and well-being of this individual. You will ensure the individual is accessing appropriate medical and dental care. Also, you will assist with nurturing positive relationships with friends, family, and support networks. This is a 24/7 position; however, additional supports will be provided and you will have access to and the support of the Home Share Coordinator.
The contracted amount for this position ranges from $880.33 to $5,015.00, with an additional $841.13 provided for room and board.
Duties include (but are not limited to):
- Assisting with showering and personal hygiene
- Meal preparation, and medication reminders
- This individual requires partial or full support in daily activities
- Supporting a client with significant mobility challenges and fall risk
Basic Requirements:
- Relevant training and/or previous experience working with individuals with diverse abilities.
- First Aid and CPR certificate.
- Valid BC driver’s license and acceptable Driver’s Abstract.
- Criminal record check.
Be willing to obtain/complete the following:
- Physician’s letter of good health.
- Three letters of reference.
- A satisfactory home-study.
- Non-Violent Crisis Intervention (NVCI) certificate.
- A signed declaration of confidentiality and a signed release of information
